Objective To investigate the clinical diagnostic value of serum levels of soluble fms-like tyrosine kinas (sFlt-1)and D-dimer (DD)in patients with preeclampsia (PE). Methods A total of 100 patients with PE who were admitted and treated in our hospital from October 2013 to May 2016 were enrolled in the study. These patients were divided into mild PE group (n = 56)and severe PE group (n = 46),at the same time,the other 34 healthy pregnant women were enrolled as control group. The general informations and laboratory parameters were recorded,and serum levels of sFlt-1 and DD were detected. ROC curve was used to evaluate the diagnostic value of sFlt-1 and DD in PE. Results There were significant differences in the serum levels of sFlt-1 and DD among the three groups with each other. The sFlt-1 levels in mild PE group were closely correlated to those of DD (r = 0. 338,P < 0. 05),and sFlt-1 levels in severe PE group were also closely associated with those of DD (r = 0. 436,P < 0. 01). The AUC of sFlt-1,DD and sFlt-1 combined with DD in differential diagnosis between patients with PE and healthy pregnant women were 0. 749,0. 722,0. 795,respectively. The AUC of sFlt-1,DD and sFlt-1 combined with DD in differential diagnosis between mild PE and severe PE were 0. 632,0. 706,0. 711,respectively. Conclusion The serum levels of sFlt-1 and DD can be regarded as reference indexes of clinical diagnosis of PE.%目的 探讨血清可溶性Fms样酪氨酸激酶-1(soluble fms-like tyrosine kinas,sFlt-1)及D-二聚体(D-Di-mer,DD)水平用于诊断子痫前期(preeclampsia,PE)的临床价值.方法 PE患者100例根据病情分为轻度PE组(n=56)及重度PE组(n=46).同时纳入34名健康孕妇作为健康对照组.所有PE患者记录一般情况及实验室指标,检测血清sFlt-1及DD水平.ROC曲线分析sFlt-1及DD水平用于PE患者的诊断价值.结果 重度PE组、轻度PE组及健康对照组.血清sFlt-1和DD水平两两比较,差异均有统计学意义.轻度PE患者sFlt-1与DD水平呈正相关(r=0.338,P=0.011),重度PE患者sFlt-1与DD水平也呈正相关(r=0.436,P=0.002).当用于区别诊断PE患者与健康孕妇时,单独使用sFlt-1的AUC为0.749,DD单独的AUC为0.722,sFlt-1联合DD的AUC为0.795.当用于区别诊断轻度及重度PE患者时,单独使用sFlt-1的AUC为0.632,单独使用DD的AUC为0.706,sFlt-1联合DD的AUC为0.711.结论 血清sFlt-1及DD水平可作为PE患者的临床诊断参考指标.
