疏血通注射液联合心室内注射人脐带间充质干细胞对急性心肌梗死大鼠心功能的影响及机制探讨

         

摘要

Objective To observe the effect of Shuxuetong injection combined with intraventricular injection of human umbilical cord mesenchymal stem cells on cardiac function in rats with acute myocardial infarction ( AMI), and to explore its mechanism. Methods 116 male SD rats were randomly divided into blank group ( 26 rats) and model group ( 90 rats). The rat models of myocardial infarction were established by subcutaneous injection of isoproterenol hydrochloride ( 200 mg/kg). 78 successful modeling SD rats were randomly divided into model group, stem cell group and combined drug group, with 26 rats in each group. In the combined drug group, rats were intraperitoneally injected with Shuxuetong injection dilution solution 5 m L/kg ( equivalent to 0. 7 m L/kg of Shuxuetong injection add once intraventricular injection of human umbilical cord mesenchymal stem cell suspension 2 m L ( 1. 0 × 10<sup>6</sup>count). In the stem cell group, rats were intraperitoneally injected with 0. 9% sodium chloride injection 5 m L/kg add once intraventricular injection of human umbilical cord mesenchymal stem cell suspension 2 m L ( 1. 0 × 10<sup>6</sup> count).Rats in the model group were intraperitoneally injected with 0. 9% sodium chloride injection 5 m L/kg add once intraventricular injection of 0. 9% sodium chloride injection 2 m L. Rats in the blank group were intraperitoneally injected with 0. 9% sodium chloride injection 5 m L/kg add once intraventricular injection of 0. 9% sodium chloride injection2 m L. The intraperitoneal injection was performed once a day, and the left ventricular ejection fraction ( LVEF), left ventricular end-systolic volume ( LVESV), and left ventricular end-diastolic left ventricular volume ( LVEDV) were measured by echocardiography in 4 groups of rats after 2 weeks of injection. The levels of lactate dehydrogenase ( LDH), creatine kinase ( CK), brain natriuretic peptide precursor ( Pro-BNP), nitric oxide ( NO), nitric oxide synthase ( NOS), tumor necrosis factor-alpha ( TNF-α) and interleukin-6 ( IL-6) in peripheral serum were measured. Results Compared with the blank group, the LVEF of model group, stem cell group and combined drug group decreased ( P < 0. 05), the LVESV and LVEDV increased ( P < 0. 05). Compared with the model group, the LVEF in the stem cell group and combined drug group increased ( P < 0. 05), the LVESV and the LVEDV decreased ( P < 0. 05). Compared with the stem cell group, the LVEF in the combined drug group increased ( P < 0. 05), and the LVESV and the LVEDV decreased ( P < 0. 05). Compared with the blank group, the serum LDH, CK and Pro-BNP in the model group, stem cell group and combined drug group increased ( P < 0. 05). Compared with the model group, the serum LDH, CK and Pro-BNP in the stem cell group and combined drug group decreased ( P < 0. 05).Compared with the stem cell group, the serum LDH, CK and Pro-BNP in the combined drug group decreased ( P <0. 05). Compared with the blank group, the serum NO, NOS, TNF-α and IL-6 in the model group, the stem cell group and the combined drug group increased ( P < 0. 05). Compared with the model group, the serum NO, NOS, TNF-α and IL-6 in the stem cell group and the combined drug group decreased ( P < 0. 05). Compared with the stem cell group, the serum NO, NOS, TNF-α and IL-6 in the combined drug group decreased ( P < 0. 05). Conclusion Shuxuetong injection combined with intraventricular injection of human umbilical cord mesenchymal stem cells can improve cardiac function by improving the expression of inflammatory factors after myocardial infarction in AMI rats.%目的观察疏血通注射液联合心室内注射人脐带间充质干细胞对急性心肌梗死 (AMI) 大鼠心功能的影响, 并探讨其作用机制.方法将116只雄性SD大鼠随机分为空白组26只及造模组90只, 造模组采用皮下多点注射盐酸异丙肾上腺素 (200 mg/kg) 制作AMI大鼠模型, 将造模成功的78只大鼠随机分为模型组、干细胞组及联合用药组, 每组26只.联合用药组大鼠予腹腔注射疏血通注射液稀释液5 m L/kg (相当于疏血通注射液0.7 m L/kg) +1次心室内注射人脐带间充质干细胞悬着液2 m L (1.0×10<sup>6</sup>个);干细胞组大鼠予腹腔注射0.9%氯化钠注射液5 m L/kg+1次心室内注射人脐带间充质干细胞悬着液2 m L (1.0×10<sup>6</sup>个);模型组大鼠予腹腔注射0.9%氯化钠注射液5 m L/kg+1次心室内注射0.9%氯化钠注射液2 m L;空白组大鼠予腹腔注射0.9%氯化钠注射液5 m L/kg+1次心室内注射0.9%氯化钠注射液2 m L.腹腔注射均每日1次, 4组大鼠干预2周后采用心脏超声测定左室射血分数 (LVEF) 、左室收缩末期容积 (LVESV) 及左室舒张末期容积 (LVEDV), 并检测外周血清乳酸脱氢酶 (LDH) 、肌酸激酶 (CK) 、脑钠肽前体 (Pro-BNP) 、一氧化氮 (NO) 、一氧化氮合酶 (NOS) 、肿瘤坏死因子α (TNF-α) 、白细胞介素6 (IL-6).结果与空白组比较, 模型组、干细胞组、联合用药组LVEF均降低 (P<0.05), LVESV及LVEDV均升高 (P<0.05);与模型组比较, 干细胞组、联合用药组LVEF均升高 (P<0.05), LVESV及LVEDV均降低 (P<0.05);与干细胞组比较, 联合用药组LVEF升高 (P<0.05), LVESV及LVEDV均降低 (P<0.05).与空白组比较, 模型组、干细胞组、联合用药组血清LDH、CK、Pro-BNP均升高 (P<0.05);与模型组比较, 干细胞组、联合用药组血清LDH、CK、ProBNP均降低 (P<0.05);与干细胞组比较, 联合用药组血清LDH、CK、Pro-BNP均降低 (P<0.05).与空白组比较, 模型组、干细胞组、联合用药组血清NO、NOS、TNF-α、IL-6均升高 (P<0.05);与模型组比较, 干细胞组、联合用药组血清NO、NOS、TNF-α、IL-6均降低 (P<0.05);与干细胞组比较, 联合用药组血清NO、NOS、TNF-α、IL-6均降低 (P<0.05).结论疏血通注射液联合心室内注射人脐带间充质干细胞能通过改善AMI大鼠心肌梗死后炎症因子的表达而改善心功能.
