参附注射液治疗心肾阳虚型慢性心力衰竭30例临床观察

摘要

目的 观察参附注射液治疗心肾阳虚型慢性心力衰竭临床疗效.方法 将60例心肾阳虚型慢性心力衰竭患者随机分为2组.对照组30例子常规西医基础治疗,治疗组30例在对照组基础上加用参附注射液治疗.2组均治疗10 d统计临床疗效,并观察2组治疗前后中医症状、中医证候积分情况及血浆脑钠肽(BNP)水平.结果 2组治疗后心悸、胸闷痛、气短、面肢水肿、畏寒肢冷、疲倦乏力、尿少、自汗、咳嗽、腹胀等程度与本组治疗前比较差异均有统计学意义(P＜0.05).治疗组在心悸、气短、面肢水肿、畏寒肢冷、疲倦乏力、自汗、咳嗽改善方面均优于对照组(P＜0.05).2组治疗后中医证候积分均较本组治疗前减少(P＜0.05),且治疗后治疗组证候积分明显低于对照组(P＜0.05).2组总有效率比较差异有统计学意义(P＜0.05),治疗组疗效优于对照组.2组治疗前后BNP均较本组治疗前降低(P＜0.01),且治疗后治疗组BNP水平低于对照组(P＜0.05).结论 参附注射液治疗心肾阳虚型慢性心力衰竭疗效确切.%Objective To observe the effects of Shenfu injection on heart failure patients with heart-kidney yang deficiency.Methods 60 heart failure patients with heart-kidney yang deficiency type were randomly divided into two groups.30 patients in control group were treated by routine western treatment.30 patients in treatment group were treated by Shenfu injection on the basis of control group treatment.The course was 10 d in two groups.The clinical effect was analyzed after a course.The traditional Chinese medicine (TCM) symptoms,TCM syndrome score and the level of brain natriuretic peptide (BNP) before and after treatment were observed in two groups.Results There were significant differences between before and after treatment on the symptoms of palpitation,chest tightness,shortness of breath,surface limb edema,extreme chilliness,fatigue,oliguria,spontaneous sweating and cough in two groups (P ＜ 0.05).The improvement of palpitation,shortness of breath,surface limb edema,extreme chilliness,fatigue,spontaneous sweating and cough in treatment group was superior to that in control group (P ＜ 0.05).The TCM syndrome score after treatment was decreased in two groups (P ＜ 0.05),and the decrease in treatment group was lower than that in control group (P ＜ 0.05).The curative effect in treatment group was superior to that in control group (P ＜ 0.05).The level of BNP after treatment was decreased in two group (P ＜ 0.05),and the decrease in treatment group was lower than that in control group (P ＜ 0.05).Conclusion Shenfu injection has an exact effect on the treatment of heart failure with heart kidney yang deficiency type.
