北京社区乳腺癌高危人群中医体质分布特点研究

摘要

目的 通过对北京西长安街地区“中国城市癌症早诊旱治项目”(以下简称“五癌筛查”)所检出的乳腺癌高危人群的中医体质调查,分析乳腺癌高危人群中医体质分布特点,为社区乳腺癌高危人群的中医健康管理、健康干预以及后续相关研究积累经验.方法 基于“五癌筛查”对筛查出的乳腺癌高危人群采用由北京市中医药管理局下发的“中医药健康管理服务软件国家标准版”软件进行中医体质辨识,并对结果进行统计分析,归纳出乳腺癌高危人群的中医体质分布特点.结果 通过“五癌筛查”纳入乳腺癌高危者173例,均为女性,体质以阳虚质、阴虚质、痰湿质居多.不同年龄段体质分布结果显示,40 ～ 49岁以气郁质为主,阴虚质为次;50～59岁以痰湿质为主,平和质为次;60 ～ 69岁以阳虚质为主,阴虚质为次.合并其他癌症高危种类数中,有6种癌症高危者以痰湿质为主,阴虚质和阳虚质为次;5种癌症高危者以阴虚质为主,平和质和湿热质为次;4种癌症高危者以阴虚质为主,痰湿质为次;3种癌症高危者以痰湿质为主,阳虚质为次;2种癌症高危者以阴虚质为主,阳虚质为次;单一乳腺癌高危者以平和质为主,痰湿质为次.结论 与乳腺癌高危关系最密切的体质偏颇类型依次是:阳虚质、阴虚质、痰湿质,且年龄段不同、合并其他癌症高危种类数不同,其体质特点也有不同,在乳腺癌高危人群的健康干预中应根据其特点制订干预对策.%Objective To analyze the distribution of traditional Chinese medicine (TCM) constitution of breast cancer high-risk people which selected from Early diagnosis and treatment of cancer in China projects (five cancer screening) in West Chang~n street of Beijing,to provide the experience of TCM healthy management,health intervention and follow-up study.Methods Using national standard TCM healt.h management services software,the TCM constitution was observed in breast cancer high-risk people which was selected from five cancer screening,analyzed the results and then generalized the distribution of TCM constitution.Results Totally 173 female were screening out,most were Yang deficiency constitution,Yin deficiency constitution and phlegm-dampness constitution.According to different ages,the most common constitution at the age between 40 to 49 was Qi stagnation,Yin deficiency second;and the 50 to 59 years old was phlegm-dampness constitution,mild physical constitution second;60 to 69 years old was Yang deficiency constitution,Yin deficiency constitution second.According to cancer high risk,the most common constitution in people with 6 cancer high risk was phlegm-dampness constitution,Yang deficiency constitution and Yin deficiency constitution second;that in people with 5 cancer high risk was Yin deficiency constitution,mild physical constitution and damp-hot constitution second;that in people with 4 cancer high risk was Yin deficiency constitution,phlegm-dampness constitution second;that in people with 3 cancer high risk was phlegm-dampness constitution,Yang deficiency constitution second;that in people with 2 cancer high risk was Yin deficiency constitution,Yang deficiency constitution second;that in people with breast cancer high risk only was mid constitution,mild physical constitution second.Conclusion The most closely TCM constitution correlated with breast cancer are,in order,Yang deficiency constitution,Yin deficiency constitution and phlegm-dampness constitution,with different constitution characteristic and cancer high risk at different age period,and the intervention strategy should be according to different constitution characteristic in breast cancer high-risk people.
