家庭医生团队专职健康教育护士对2型糖尿病患者健康教育效果评价

摘要

目的：探讨家庭医生团队中专职健康教育护士对2型糖尿病患者健康教育效果及重要指标的影响。方法：选取社区健康服务中心慢性病管理中的2型糖尿病患者符合入选条件96名，以自愿为原则签约服务。对比干预前后空腹血糖、餐后2h 血糖、糖化血红蛋白、体重指数数据。结果：干预12个月后，空腹血糖、餐后2h 血糖、糖化血红蛋白、体重指数明显改善。差异具有统计学意义（p ＜0.05）。结论：采用家庭医生团队专职健康教育护士对2型糖尿病患者进健康教育，能影响患者生活方式，明显改善患者多项重要指标。对慢性病管理及家庭医生团队建设具一定指导意义。%In this paper Purpose:Family doctor on the team full -time health education nurse in patients with type 2 diabetes health education effect and the influence of the important indexes.Methods:Selection of community health service center of chronic disease management of patients with type 2 diabetes conditions for 96, based on principles of voluntary signing service.Contrast before and after the intervention in fasting plasma glucose,2 h postprandial blood glucose,glycosylated hemoglobin,body mass index data.The results of:12 months after intervention,fasting blood sugar,2 h postprandial blood glucose,glycosylated hemoglobin, body mass index improved significantly.The difference is statistically significant(p <0.05).By the family doctor team health professional education nurses into the health education in patients with type 2 diabetes,can affect patients'lifestyle,significantly improve patients with a number of important indicators.Manage-ment of chronic illness and family doctor the construction team has a certain guiding significance.
