Objective To observe effect of external fixation in the treatment of severe tibia and fibula open fracture. Methods 56 cases with severe tibia and fibula open fracture from 2008 to 2010 under going external fixator were selected as experience group. And 48 patients under going open reduction and internal fixation were considered as the control group. Clinical effect, operation time, intraoperative hemorrhages, hospital stay, the time of bone healing and complications rate were compared between the two groups. Results The excellent and good rate were 87.5% and 79.2% in experience group and control group, respectively, with significant difference (P<0.05); there were no statistically significant difference in operation time, intraoperative hemorrhage and hospital stay between the two groups; the time of bone healing in experience group was much shorter than that in control group (P<0.05); Complications rate were 7.1% and 22.9% in experience group and control group (P<0.01). Conclusion External fixation is very effective in the treatment of severe tibia and fibula open fracture, with ideal reduction, firm fixation, less complications and faster recovery.%目的 观察外固定架治疗胫腓骨严重开放粉碎性骨折的应用效果.方法 选取2008-2010年本院住院治疗的56例胫腓骨严重开放粉碎性骨折患者作为观察组,采用外固定架治疗.以2005-2007年住院治疗的48例胫腓骨严重开放粉碎性骨折患者作为对照组,采用切开复位内固定组治疗.比较两组临床疗效、手术时间、术中出血、住院时间、骨性愈合时间及并发症发生率.结果 观察组与对照组优良率分别为87.5％和79.2％,差异具有统计学意义(P＜0.01)；两组手术时间、术中出血及住院时间差异无统计学意义(P＞0.05)；观察组骨性愈合时间显著少于对照组(P＜0.05)；观察组并发症发生率为7.1％,显著低于对照组的22.9％ (P＜0.01).结论 外固定架治疗胫腓骨严重开放粉碎性骨折复位理想,固定牢固,患者术后并发症少、肢体功能恢复快.
