Chronic diseases have been developed into the major diseases that affect human health. Chronic dis-eases persist for a long time, and generally cannot be cured by medication. The prevalence and mortality rates of chronic diseases tend to become more common with age, which bring heavy burdens to individuals, families and the society. It is imminent to carry out the chronic disease management in general hospital. Today, only in the way of updating the medi-cal treatment philosophy, exploring the new ideas through combined new technologies and methods, and seeking innova-tions, the management of chronic disease can be able to keep pace with the times.%慢性病已发展成影响人类健康的主要疾病，其病程长且病情多迁延不愈，患病率和死亡率不断上升，给个人、家庭、社会带来沉重负担，做好综合医院的慢性病管理已迫在眉睫。现今，只有更新医疗理念，结合新技术、新方法，寻找新的思路，创新慢性病管理模式才能适应时代的步伐。
