血清Fe和血小板检测对结核病活动性判断的临床价值

             

摘要

目的 评价血清铁离子(Fe)和血小板水平检测对结核病活动性的临床判断价值.方法 选择2012年2月至2015年7月在梧州市第三人民医院门诊或住院的患者,其中活动性结核病患者、非活动性结核病患者以及健康志愿者各78例.对活动性结核病组治疗前和治疗后3个月,非活动性结核病患者和健康组人群纳入时分别检测血清Fe、血小板水平、白细胞计数等指标,最后采用相关性分析法评价结核病活动性与血清Fe水平、血小板以及白细胞之间的相关性.结果 各组受试者的血清Fe水平显示为活动性结核病组治疗前<活动性结核病组治疗后3个月<非活动性结核病组<健康组,而血小板、白细胞计数水平显示为活动性结核病组治疗前>活动性结核病组治疗后3个月>非活动性结核病组>健康组,差异均有统计学意义(P<0.05).经相关性分析显示,结核病活动性与血清Fe水平存在显著的负相关(r=-0.451),与血小板(r=0.792)、白细胞(r=0.837)存在显著正相关.结论 活动性结核病患者血清中Fe水平明显更低,而血小板水平更高.两者与病情的活动性具有显著的相关性,加上白细胞计数等水平的同时监测对活动性判断更为准确.%Objective To evaluate the clinical value of detection of serum iron ion (Fe) and platelet level in de-termining the activity of tuberculosis (TB). Methods Patients in our hospital from February 2012 to July 2015 were en-rolled in the study, including 78 patients with active TB (active TB group) and 78 patients with inactive TB (inactive TB group). Seventy-eight healthy volunteers were selected as the control group. Serum Fe, platelet count, white blood cell count were detected in active TB group before and after 3 months of treatment, as well as in inactive TB group and con-trol group. Correlation analysis was performed to evaluate the correlation between the activity of TB and serum Fe level, platelet and white blood cells. Results Serum Fe level increased successively in active TB group before treatment, ac-tive TB group after treatment, inactive TB group, and control group, but the platelet, white blood cell count were de-creased successively. The differences were statistically significant (P<0.05). Correlation analysis showed that the activity of tuberculosis was negatively correlated to serum Fe level (r=-0.451), and positively correlated to platelet (r=0.792) and white blood cell (r=0.837). Conclusion Patients with active tuberculosis have significantly lower serum Fe level and higher blood platelet levels. Detection of serum Fe and platelet, in combination of white blood cell, has great value in evaluating the activity of tuberculosis.
