目的 探讨社区护理干预对膀胱造瘘患者生活质量的影响.方法 选择2015年1月至2016年3月期间碧海云天社区膀胱造瘘术后出院的长期带管患者58例为研究对象,根据随机数表法分为观察组(n=29)与对照组(n=29).对照组患者予常规出院指导,观察组患者予社区护理干预,干预时间为1年.比较两组患者并发症发生率及生活质量情况(JQOLI-74量表评分).结果 干预1年后,观察组患者并发症发生率为6.90％,明显低于对照组的24.14％,差异有统计学意义(P<0.05);观察组患者JQOLI-74量表中的心理功能、躯体功能、社会功能、物质生活等四个维度及总分分别为(64.3±8.9)分、(63.9±7.4)分、(60.2±5.8)分、(55.3±4.2)分、(241.4±24.6)分,均明显高于对照组的(53.4±6.5)分、(52.0±7.1)分、(53.1±4.5)分、(51.2±3.6)分、(207.2±18.2)分,差异均有统计学意义(P<0.05).结论 对长期留置膀胱造瘘管患者进行社区护理干预,可明显降低造瘘管相关的并发症发生率,提高患者的生活质量.%Objective To explore the effect of community nursing intervention on the quality of life in patients with bladder stoma. Methods Fifty-eight patients with long-term bladder stoma discharged from Bihaiyuntian Commu-nity Health Service Center between January 2015 and March 2016 were selected as research objects. According to the random number table, the patients were randomly divided into observation group (n=29) and control group (n=29). The control group received routine discharge guidance, while the observation group was given community nursing interven-tion for 1 year. The incidence of complications and quality of life (JQOLI-74 scale score) were compared between the two groups. Results After the intervention of 1 year, the complication rate of observation group was 6.90％, significant-ly lower than 24.14％ in the control group (P<0.05). The JQOLI-74 scale score of mental function, physical function, so-cial function, material life, and total score were respectively (64.3±8.9), (63.9±7.4), (60.2±5.8), (55.3±4.2), (241.4±24.6) in observation group, significantly higher than (53.4±6.5), (52.0±7.1), (53.1±4.5), (51.2±3.6), (207.2±18.2) in the control group (P<0.05). Conclusion The community nursing intervention for patients with long-term indwelling bladder stoma can significantly reduce catheter-associated complications and improve the quality of life.
