视频反馈教学法结合中医按摩应用于全髋置换术后功能康复的效果评价

             

摘要

Objective To evaluate the effect of video feedback teaching method combined with traditional Chi-nese medicine massage on patients and their family members after total hip arthroplasty. Methods A total number of 150 patients with total hip arthroplasty in the Fourth Department of Orthopedics, Zhongnan Hospital of Wuhan Universi-ty from January 2013 to December 2014 were selected. There were divided into three groups (Group A, Group B, Group C) according to the random number table, with 50 patients in each group. Ther group A (Control group) was trained and exercised by nurses at the bedside to perform postoperative routine functional exercise guidance and practice. The group B was used of Chinese Medicine massage on the basis of the. The group C was routine exercise combined with Chinese massage for routine functional exercise, and the use of "practice-VIDEO-feedback" video feedback teaching method for functional exercise guidance. The hip joint function recovery, functional training, the patient's family involvement, pa-tient satisfaction, postoperative complications and average guidance time of nurses during hospitalization were compared and statistically analyzed among the three groups. Results 1 months after operation, the score of articulatio coxae in group B was 66.0%, which was significantly beteer than 60.0% in group A, and the group C (70.0%) was better than group B, and the differences were statistically significant (P<0.05). 3 months after the operation, the score of articulatio coxae in group B was 84.0%, which was significantly beteer than 80.0%, and the group C (92.0%) was better than group B, and the differences were statistically significant (P<0.05). The group C (excellent rate of 94.0%, satisfaction 98.0%) and group B (excellent rate of 88.0%, satisfaction 92.0%) of of patients related to functional exercise skills and patient satisfaction was higher than A group (excellent rate of 80.0%, satisfaction 88.0%), and the differences were statistically significant (P<0.05). The average guidance time of nurses of group B and group C were respectively (3.58±1.21), (2.88± 0.97), which were significantly less than (4.36±0.73) of control group, and the group C was significantly less than that in group B (P<0.05). Conclusions Combination of traditional Chinese medicine and Western medicine in the guidance of video feedback teaching is more beneficial to the rehabilitation of hip joint replacement, and it is worth to be popularized and used.%目的 评价视频反馈教学法联合中医按摩应用于指导患者及家属全髋关节置换术后患者功能康复的效果.方法 选取2013年1月至2014年12月武汉大学中南医院骨四科收治的150例行全髋关节置换术患者,按照随机数表法分为A、B、C三组各50例;A组为对照组,由护士在床边对患者进行术后的常规功能锻炼指导并练习;B组在对照组的基础上运用中医按摩护理;C组为常规功能锻炼结合中医按摩护理,并采用"练习-拍摄视频-反馈"的视频反馈教学法进行功能锻炼指导.对三组患者髋关节功能恢复、患者功能锻炼掌握情况、患者家属参与情况、患者满意度、患者术后并发症和住院期间护士平均指导时间进行比较和统计学分析.结果 术后1个月,B组(66.0%)患者髋关节评分优于A组(60.0%),且C组(70.0%)患者髋关节功能评分优于B组,差异均有统计学意义(P<0.05);术后3个月,B组(84.0%)患者髋关节评分优于A组(80.0%),且C组(92.0%)患者髋关节功能评分优于B组,差异有显著统计学意义(P<0.01);C组(优良率为94.0%,满意度为98.0%)和B组(优良率为88.0%,满意度为92.0%)患者功能锻炼相关技能的掌握情况及患者满意度高于A组(优良率为80.0%,满意度为88.0%),差异有统计学意义(P<0.05);B组(3.58±1.21)h和C组(2.88±0.97)h护士的平均指导时间明显少于对照组(4.36±0.73)h,并且C组护士平均指导时间明显少于B组,差异具有统计学意义(P<0.05).结论 视频反馈教学指导下的中西医结合康复锻炼更加有利于髋关节置换术后康复.
