老年急性胆源性胰腺炎早期EST的治疗

摘要

Objective To explore elderly paitents with acute billary pancreatitis early EST treatment curative effect. Method Retrospective analysis the clinical data of 38 cases of old age ABP early EST treatment from 2001-2011, and non-endoscopic treatment group in 41 cases of old age ABP during 1993-2000 carries on the paired observation. Results With the non-endoscopic treatment group compares, the endoscopic treatment group in body temperature return to normal time, abdominal pain away time, serum amylase and WBC dropped to normal time and length of stay are significantly reduced (P<0.05), light ABP heavy proportion, the complication rate and mortality rate has dropped markedly (P<0.05). Conclusion The early EST treatment of old age ABP curative effect is satisfied, can relieve the obstruction promptly, blocks the pancreatitis heavy tendency, especially suitable for elderly patient (merge disease more and can’t tolerate the operation).%目的：探讨老年急性胆源性胰腺炎早期内镜下十二指肠乳头括约肌切开治疗的疗效。方法回顾性分析我院2001年至2011年间38例老年ABP早期EST治疗临床资料，并与1993年至2000年间非内镜治疗组41例老年ABP进行对比观察。结果与非内镜治疗组相比，内镜治疗组在体温恢复正常时间、腹痛消失时间、WBC及血淀粉酶下降正常时间以及住院时间均明显缩短（P<0.05），轻型ABP重型化比例、并发症发生率及病死率的发生均明显下降（P<0.05）。结论早期EST治疗老年急性胆源性胰腺炎疗效满意，能及时解除梗阻，阻断胰腺炎重型化趋势，尤为适合老年合并病多、难以耐受手术的患者。
