微创经皮肾镜手术及新型经皮肾镜手术联合治疗巨大鹿角形肾结石的效果研究

摘要

目的 探讨微创经皮肾镜手术联合新型经皮肾镜手术对巨大鹿角形肾结石的临床治疗效果.方法 对我院2007年1月至2012年6月收治的142例巨大鹿角形肾结石患者的临床治疗情况进行回顾性分析，根据患者接受的手术治疗方式将其分为观察组与对照组.观察组共75例患者，给予微创经皮肾镜手术及新型经皮肾镜手术联合治疗；对照组共67例患者，给予微创经皮肾镜手术治疗.比较两组患者手术时间、术中出血量、平均住院时间以及结石取净情况.结果 观察组患者平均手术时间及平均住院时间显著少于对照组，差异具有统计学意义(P<0.05)；观察组患者1次取净结石率及结石总清除率显著高于对照组，差异具有统计学意义(P<0.05)；两组患者均未出现严重并发症.结论 对巨大鹿角形肾结石患者给予微创经皮肾镜手术与新型经皮肾镜手术联合治疗，安全性高、创伤小、手术时间短、术中出血量少、结石清除率高、术后并发症少的优点，临床治疗效果显著.%Objectives To observe the clinical effect of minimally invasive percutaneous nephrolithotomy operation combined with new percutaneous nephroscope operation to treat giant renal staghorn calculi. Methods The clinical treatment of 142 patients with giant renal staghorn calculi treated in our hospital from January 2007 to June 2012 were analyzed retrospectively and divided into 2 group according to operation treatment methods received by patients. There were 75 patients in the observation group which were treated with minimally invasive percutaneous nephrolithotomy operation combined with new percutaneous nephroscope operation. There were 67 patients in the control group which were treated with minimally invasive percutaneous nephrolithotomy operation. Comparing the operation time, the amount of bleeding in operation, the average hospitalization time and net extraction condition of 2 groups. Results As for the average operation time and the average hospitalization time, the observation group was less than the control group, which had statistical significance(P<0.05).1 remove the stone rate and stone total clearance of the observation group were higher evidently than that of the control group, which had statistical significance(P<0.05).There was no severe complications in 2 groups. Conclusions Minimally invasive percutaneous nephrolithotomy operation combined with new percutaneous nephroscope operation to treat giant renal staghorn calculi has the advantages of high safety, small wound, short operation time, less bleeding in operation, high stone clearance rate and less postoperative complications, and the clinical therapeutic effect is significant.
