单髁置换术治疗不同年龄段中老年单侧膝内侧间室骨性关节炎患者的早期疗效

摘要

目的 观察不同年龄段中老年单侧膝内侧间室骨性关节炎患者行单髁置换术(UKA)的早期临床疗效.方法 回顾性分析169例行UKA治疗的单侧膝内侧间室骨性关节炎患者临床资料,按年龄分为高龄组( >74岁)、老年组(60~74岁)及中年组( <60岁).观察3组手术所需时间、术中出血量、术后住院时间及术后并发症发生情况,术后随访2年,采用美国特种外科医院(HSS)膝关节评分、视觉模拟量表(VAS)评分及膝关节最大屈曲度评估3组临床疗效.结果 3组患者手术时间、术后住院时间及术后并发症发生率比较,差异均无统计学意义(均P>0.05);中年组出血量均多于老年组及高龄组(均P<0.05),而老年组与高龄组比较,差异无统计学意义(P>0.05).末次随访时,3组患者HSS评分及膝关节屈曲度均高于术前,VAS评分均低于术前(均P<0.05),而3组间HSS评分、VAS评分及膝关节屈曲度比较,差异均无统计学意义(均P>0.05).结论 UKA治疗不同年龄组段的单侧膝内侧间室骨性关节炎患者早期疗效相当.%Objective To observe the clinical effect of unicompartmental knee arthroplasty(UKA) on the middle-aged and elderly patients with medial unicompartmental knee osteoarthritis in different age groups.Methods The clinical data of 169 patients with medial unicompartmental knee osteoarthritis who underwent UKA were retrospectively analyzed.And the patients were divided into very elderly group(older than 74 years),elderly group(aged 60 to 74 years) and middle-aged group(younger than 60 years).The operation duration, intraoperative blood loss,postoperative hospital stay and incidence of postoperative complications were observed in the three groups.After two years of follow-up,the Hospital for Special Surgery(HSS) Knee Score,Visual Analogue Scale(VAS) score and maximum knee flexion were used to assess the clinical efficacy in the three groups.Results There was no significant difference among the three groups in the operation duration,postoperative hospital stay or incidence rate of postoperative complications(all P>0.05);the blood loss in the middle-aged group was more than that in the elderly group or the very elderly group(P<0.05),but no significant difference was observed between the elderly group and the very elderly group(P>0.05).During the final follow-up,the HSS scores and knee flexion were higher while the VAS scores were lower than the preoperative among the three groups(all P<0.05),and there were no significant differences in the HSS score,VAS score or knee flexion among the three group(all P>0.05).Conclusion UKA has similar early-stage efficacies for treating medial unicompartmental knee osteoarthritis in different age groups.
