风湿性心脏病二尖瓣狭窄心房颤动与窦性心律心肌细胞起搏电流密度的差异

     

摘要

Objective To analyze the difference in the funny current ( If ) densities between the atrial fibrillation ( AF ) and sinus rhythm ( SR ) in human atiral mycaridoocytes from patients with rheumatic mitral stenosis. Methods By acute separation method, the human atrial myocaridocytes were divided into Group AF and Group SR. The If densities were recorded for comparison. Results With the test potential between -40 mV and - 140 mV, there were significant differences in the funny current densities between the Group SR and Group AF ( P <0. 01 ). And the difference reached peak [ ( -42. 98 ±2. 77 ) vs ( - 26. 93 ± 1. 08 ) pA/pF ] with the test potential of - 70 mV. Conclusion The funny current density in AF in human atiral myocaridocytes is larger than that in SR, suggesting that the increase of funny current density may lead to AF, thus contributes to the pathogenesis of AF in rheumatic mitral stenosis.%目的 运用全细胞膜片钳技术,研究风湿性心脏病二尖瓣狭窄心房颤动(简称房颤)与窦性心律(简称窦律)心肌细胞起搏电流(If)的差异.方法 急性分离人心肌细胞,将其分为风湿性心脏病二尖瓣狭窄房颤(atrial fibrillation,AF)组和窦律(sinus rhythm,SR)组,记录两组的If,观察两组心肌细胞If的差别.结果 AF组心肌细胞的If密度明显大于SR组,在-140~-40 mV,AF组和SR组电流密度的差异有统计学意义(P<0.01);其中,在-70 mV时,AF组与SR组If的电流密度分别为(-42.98±2.77)与(-26.93±1.08)pA/pF,此时两组电流密度差异最大.结论 AF心肌细胞的If密度较SR心肌细胞增大,提示If增大可能导致AF,可能是风湿性心脏病二尖瓣狭窄房颤发生的机制之一.
