经阴道彩色多普勒超声对瘢痕子宫妊娠的诊断价值

摘要

Objective To investigate diagnostic value of cesarean scar pregnancy by transvaginal color Doppler ultrasound. Methods 19 cases of uterine scar pregnancy of our hospital in April 2009 to October 2012 were analyzed retrospectively , according to the sonographic features and internal blood fow before and after treatment. Results Of the 19 patients, eight cases of anterior scar at the lower uterine segment shows the gestational sac, 11 cases of mixed mass. Mass rich blood fow signals were seen around the gestational sac, low resistance fow spectrum. Two cases of misdiagnosis of 19 patients, 1 patient gestational sac deformation, more depth to the uterine scar at implantation position is not clear. One cases of ultrasound diagnosis of trophoblastic disease. Conclusion The most direct and effective method of uterine scar pregnancy is by transvaginal color Doppler ultrasound.%　　目的探讨经阴道彩色多普勒超声对剖宫产后瘢痕处妊娠的诊断价值。方法对2009年4月-2012年10月19例瘢痕子宫妊娠，根据治疗前后的声像图特点及内部血流情况进行回顾性分析。结果19例患者中8例为子宫下段前壁瘢痕处显示孕囊，11例显示为混合性包块。包块及孕囊周围均可见丰富血流信号，呈低阻血流频谱。19例患者中误诊2例，1例患者孕囊变形，向宫腔深入较多，于瘢痕处着床位置显示不明确。1例超声诊断为滋养细胞病变。结论经阴道彩色多普勒超声是瘢痕子宫妊娠最直接最有效的检查方法。
