目的：探讨实施早期疼痛护理对食管癌患者术后疼痛的缓解效果。方法对2011年2月至2013年2月于我院实施手术治疗的60例食管癌患者进行随机分组，分为对照组和观察组，每组30例。对照组实施常规护理，观察组在常规护理的基础上，进行早期疼痛护理，通过VAS评分法及患者睡眠时间、住院时间等因素，综合分析比较两组护理效果。结果两组患者VAS评分在术后24、48、72 h时，差异均具有统计学意义（P<0.01）。在以上时间段观察组轻度疼痛例数分别为22例、25例、29例，分别占总病例的73.3%，83.3%，96.7%；对照组轻度疼痛为12例、16例、19例，分别占总病例40.0%、53.3%、63.3%；另外，观察组平均睡眠时间（7.9 h）多于对照组（5.3 h）；且观察组术后72 h需使用镇痛药者仅1例，而对照组为8例；观察组满意度为98.5%，高于对照组的83.2%。结论早期疼痛护理能有效缓解食管癌患者术后的疼痛程度，提高患者治疗依从性及患者满意度，值得临床上推广应用。%Objective To investigate the implementation of early pain care in patients with esophageal cancer for pain relief. Methods Eighty patients with esophageal cancer from February 2011 to February 2013 in our hospital were selected and randomly devided into two groups, 30 in each group. The control group received the usual care, and the observation group had early pain care based on the usual care. The comprehensive analysis and comparison of VAS score, patient sleep time, hospital stay and other factors were studied in this research. Results The differences of VAS score in the both two groups of patients in 24, 48, 72 h after operation were statistically significant (P<0.01). The mild pain cases in the observation group was 22, 25, 29, which accounted for 73.3%, 83.3%, 96.7%of the total cases respectively;while the mild pain cases of the control group were 12, 16, 19, which accounted for 40% 53.3%, 63.3%. In addition, we found that the average sleep time of the observation group was 7.9 h which was longer than 5.3 h of the control group. Only one case in the observation group needed to use analgesics after 72 h of the operation, while 10 cases in the control group. The patients’satisfaction in the observation group was 98.5 % which was higher than 83.2% in the control group. Conclusion Early pain care can effectively relieve the postoperative pain in patients, improve treatment compliance and satisfaction of patient, which is worth of clinical promotion.
