目的 探讨带关节外固定支架结合有限内固定治疗桡骨远端粉碎性骨折的临床效果.方法 回顾性分析本院2006年1月至2008年4月采用带关节的外固定支架结合克氏针有限内固定治疗桡骨远端粉碎性骨折29例的临床资料.结果 29例均获随访7~23个月,平均12个月.术后测量掌倾角8°~17°,尺偏角16°~26°,骨折完全愈合时间10~13周,平均10.3周.腕关节功能按Sarmiento标准评定为优19例,良7例,可3例.放射学参照Lidstrom评价为优良的为89.7%.结论 带关节外固定支架结合克氏针内固定治疗桡骨远端粉碎性骨折可以较好地恢复腕关节面的平整,维持桡骨远端关节内骨折的稳定,具有复位良好、固定可靠、疗效满意、并发症少等优点.%Objective To explore the clinical efficacy of dynamic articulars panning external fixation combined with finite internal fixation on the distal radius comminuted fractures. Methods 29 patients with distal radius comminuted fractures treated with dynamic articulars panning external fixation combined with finite internal fixation were analyzed retrospectively. Results Followed up the 29 cases for 7-23 months, 12 months as the average. After operation, the angle of metacarpal obliquity was 8°～17°, the angle of metacarpal was 16°～26°. Completely union of fracture was 10-13 weeks,10.3weeks as the average. According to Satmieno criteria, 19 cases were graded as great, 7 cases were graded as good,and the rest 3 cases as fair. according to Lidstrom of radio appreciation,the excellent rate was 89.7％. Conclusion Dynamic articulars panning external fixation combined with internal fixation with Kirschner wire can make the surface of the wrist joint recover, and the external fixation can maintain the stability of the intra-articular distal radius fracture,a combination of both treatments for comminuted distal radius fractures can have the following advantages,with good reduction,reliable,a satisfactory result and fewer complication.
