黄芪注射液对脓毒症大鼠急性肾损伤的保护作用

     

摘要

目的 探讨黄芪注射液对脓毒症大鼠急性肾损伤(AKI)的作用并探讨其机制.方法 将72只SD(Sprague Dawley)雄性大鼠随机分为假手术组、模型组和黄芪组,每组大鼠24只.采用盲肠结扎穿孔法建立大鼠脓毒症AKI模型,黄芪组术后予以黄芪注射液干预.各组在术后8、24 h各取12只大鼠,检测血清胱抑素C(Cys C)水平、肾组织匀浆中肿瘤坏死因子(TNF)-α和白介素(IL)-1β水平以及核因子(NF)-κB活性,并观察肾脏病理学变化.结果 模型组大鼠肾小管上皮细胞变性、坏死,肾小管囊腔扩张,肾间质炎症细胞浸润,术后24 h病变较术后8 h重;黄芪组病变较模型组减轻.在两个时间点,模型组大鼠的血清Cys C水平、肾组织TNF-α、IL-1β水平和NF-κB活性均高于假手术组,而黄芪组均低于模型组(P<0.05).结论 黄芪注射液通过抑制肾组织NF-κB活性、下调TNF-α和IL-1β水平,减轻脓毒症大鼠AKI.%Objective To explore the protective effect of astragalus injection on acute kidney injury( AKI) induced by sepsis in rats and its mechanism.Methods 72 male SD(Sprague Dawley) rats were randomly divided into sham operation group, model group and astragalus group with 24 in each group.AKI models of rat induced by sepsis were established by cecum Iigation and perforation.Rats in astragalus group were treated with astragalus injection after operation.Detection of serum concerntration of Cystatin-C(Cys C) , level of tumor necrosis factoralpha(TNF-α) and interleuklin-1 beta(IL-1β) , and activity of nuclear factorkappa B(NF-κB) in homogenate of renal tissues as well as observation of renal pathological changes of 12 rats in each group were conducted at 8 and 24 hours after operation.Results In model group,degeneration and necrosis of renal tubular epithelial cell, dilatation of renal tubular cavity and inflammatory cell infiltration in renal interstitial were observed, and renal pathological changes at 24 hours were more severe compared to those observed at 8 hours after operation.Renal pathological changes were observed to be milder in the astragalus group than those in the model gorup.Serum concentrations of Cys C,levels of TNF-α and IL-1β,and activity of NF κB in renal tissues at both 8 and 24 hours after operation in model group were higher than sham operation group,while those in as tragalus group were lower than those in model group(P<0.05).Conclusion Astragalus injection can alleviate AKI induced by sep sis in rat via down-regulating levels of TNFa and IL-1β and inhibiting of activity of NF-κB in renal tissues.
