专病一体化管理在控制腹膜透析患者容量超负荷中的应用

摘要

目的：探讨专病一体化管理在控制腹膜透析患者容量超负荷中的应用效果。方法：探索性使用专病一体化管理，运用持续质量改进理念对腹膜透析各个环节和透析相关合并症等进行反复评估、整改。结果：患者高血压控制达标率、体重监测达标率明显上升；因心功能不全及高血压等因素退出腹膜透析的患者比例由2008年的31.0％下降至2011年的4.0％，因容量超负荷导致患者死亡的发生率由2008年的23.5％下降至2011年的11.8％。结论：应用专病一体化管理有效降低了腹膜透析患者容量超负荷相关合并症的发生率，提高了患者的生活质量。%Objective:To investigate the effects of professional integration management program on controlling volume overloaded peritoneal dialysis (PD) patients. Methods:On the base of continuous quality improvement theory, the professional integration management program was used to manage the peritoneal dialysis patients;existing problems was found out and then the plan was improved and re-executed. Results:The rates of high blood pressure and over-weight were significantly decreased. PD patients quitted treatment as for the heart failure, hypertension, were descended from 31.0%in 2008 to 4.0%in 2011;the mortality led by volume overload descended from 23.5%in 2008 to 11.8%in 2011. Conclusion:Professional integration management program effectively reduced the incidence of volume overload and improve the patients' quality of life.
