经胸动态三维超声心动图的定性和定量临床研究

     

摘要

目的:探讨采用磁场定位系统经胸三维超声心动图(3DE)评价心血管疾病的定性和定最研究。方法:采用HP SONOS 5500超声显像仪、S4探头及其磁场定位装置和TOMTEC ECHO SCAN 三维重建工作站,对38例研究对象进行经胸超声心动图三维重建,其中男性21例,女性17例,平均年龄(40.4±16.2)岁,其中14例与手术结果对照,其中 3例与经食管超声三维重建结果对照。受检者均分别采集30及60幅图像(3D30和 3D60)各自进行三维重建和测定左室射血分数(LVEF)。结果:1、应用扇形扫描法采集图像进行三维重建时,所有三维图像均完整真实,内膜及心腔内结构清晰可辨。经胸超声三维重建显示的结构与手术和经食管超卢三维重建的结果非常接近;2、经胸超声心动图三维重建的图像采集时间为(152.55±53.27)秒,后处理时间为3~6分钟;3、3D60及3D30测得的LVEF与 2D的测值显著相关。3D30的采集时间比3D60的时间节省48%。结论:磁场定位系统扇形扫描法经胸三维超声心动图能够真实可靠迅速地显示心脏的空间结构,为心血管疾病的定性和定量诊断提供无创伤性的新方法。%Objective: To qualitatively and quantitatively study some cardiovascular diseases with transthoracic three-dimensional echocardiography (3DE). Methods: HP SONOS 5500 imaging system with a S4 probe, a magnetic tracking system,and Tomtec Echo Scan workstation were employed to examine 38 subjects. Left ventricualr ejection fraction (LVEF) were acquired by using 30 and 50 slices (3Da0 and 3D60). Results: 1. The images of 3D60 and 3D30by sectional scanning method were clear, and cardiac structures could be well defined. The images of 3D60 and 3D30 were similar to those by transesophageal three-dimensional echocardiography and operative findings. 2. The 3DE time was 152.55± 53.27 seconds, and the postprocessing time were 3-6minutes. LVEF from 3D30 and 3D60 were well correlated with that from two-dimensional echocardiography. The 3Da0 acquisition time was 48% less than the 3D60 acquisition time. Conclusions: Transthoracic 3DE with a magnetic tracking system is a new modality to evaluate cardiovascular diseases qualitatively and quantitatively.
