目的 探讨应用应变-血压指数评价糖尿病合并高血压患者胫前动脉弹性改变的特点.方法 病例分为4组:糖尿病合并高血压患者30例(DM+EH组),糖尿病患者30例(DM组),高血压患者30例(EH组),健康对照组30例(NC组),对所有被检者右侧胫前动脉进行应变和应变率成像(Xstrain),获得胫前动脉内膜的最大圆周应变,同时测量被检查段动脉的血压,计算应变-血压指数,然后各组间进行对比分析.结果 4组胫前动脉应变-血压指数差异存在统计学意义(P<0.05),DM+EH组<DM组<EH组<NC组;相关分析结果显示糖尿病合并高血压患者胫前动脉的应变-血压指数与收缩压、舒张压、空腹血糖、糖化血红蛋白呈负相关(r=-0.609,P=0.001;r=-0.512,P=0.001;r=-0.547,P=0.001;r=-0.671,P=0.001).结论 糖尿病合并高血压患者胫前动脉弹性改变较糖尿病患者和高血压患者明显,应变-血压指数可用于评价中型动脉弹性.%Objective To explore the features of anterior tibial arterial elasticity in diabetes patients accompanied by hypertension by strain-blood pressure index. Methods One hundred and twenty patients were divided into four groups: diabetes accompanied hypertension group ( DM + EM, n = 30), diabetes group ( DM, n = 30 ), hypertension group ( EM, n = 30), normal control group ( NC, n = 30) . Maxmum of circumferential strain (CSmax) of anterior tibial artery was acquired through X-strain in all the subjects. Local blood pressure of the arteries was also measured. Then strain-blood pressure index (SBPI) was calculated. Results There were significant differences between SBPI of brachial artery among the four groups, EM+ DM<DM< EH< NC. Systolic pressure, diastolic pressure, blood glucose, glycosylated hemoglobin were negatively correlated with SBPI of anterior tibial artery (r= -0. 609, P = 0. 001. r= -0.512,P= 0. 001. r= - 0. 547, P = 0. 001. r = - 0. 671, P = 0. 001 ) in diabetes accompanied hypertension group.Conclusions The elasticity of anterior tibial artery was more seriously impaired in diabetes patients accompanied by hypertension than that in patients with hypertension and patients with diabetes. SBPI can be used in assessing the elasticity of medium-sized arteries.
