实时全容积心脏成像定量评价正常早期新生儿左室容积与收缩同步性

     

摘要

Objective To assess left ventricular volume and systolic synchronicity in early newborn by real-time full-volume echo-cardiac imaging. Methods Eighty-nine early newborns were examined. Real-time full-volume echo-cardiac imaging was performed to evaluate LV systolic volumetric parameters, including end-systolic volume (ESV), end-diastolic volume(EDV), stroke volume(SV),ejection fraction(EF) , end systolic sphericity index(ESSI) , end dias-tolic sphericity index(EDSI) and synchrony parameters, including systolic dyssynchrony index(SDI), dispersion end systole(DISPES) ,mean end systolic time(MES) , Pre- contraction time volume(Precontr) and Post- contraction time volume(Postcontr). Results Although these parameters including ESV.EDV, SV, EF, ESSI and EDSI with PDA group were greater than those without PDA group, and these parameters including SDI, DISPES,MES, Precontr and Postcontr with PDA group were smaller than those without PDA group, there are not significant differences between two groups. Conclusions Real-time full-volume echo-cardiac imaging can objectively and quantitatively evaluate left ventricular volume,function and systolic synchrony in early newborn.%目的 初步探讨实时全容积心脏成像评价正常早期新生儿左室容积与收缩同步性的价值.方法 选取89例正常早期新生儿进行实时全容积心脏成像,获取左室收缩期容积参数:收缩末期容积(ESV),舒张末期容积(EDV),每搏输出量(SV),射血分数(EF),收缩末期球形指数(end systolic sphericity index,ESSI)及舒张末期球形指数(end diastolic sphericity index,EDSI);左室收缩同步性参数:收缩期失同步性指数(systolic dyssynchrony index,SDI),收缩末期离散度(dispersion end systole,DISPES),平均收缩末期时间(mean end systolic time,MES),收缩前时间容积(Precontraction time volume,Precontr),收缩后时间容积(Post-contraction time volume,Postcontr).结果 动脉导管未闭组的左室舒张末期容积,每搏输出量、射血分数、收缩末期球形指数及舒张末期球形指数均要大于动脉导管闭合组.同样,动脉导管未闭组的左室收缩失同步指数、收缩末离散度、平均收缩末时间、收缩前时间容积及收缩后时间容积较动脉导管闭合组有减小的趋势,但两组之间并没有显著性差异.结论 实时全容积心脏成像能够客观、定量的评价左室容积、功能及左室收缩同步性.
