经会阴实时三维超声在女性后盆腔功能障碍所致便秘中的诊断价值

摘要

目的 探讨经会阴实时三维超声对女性后盆腔功能障碍所致便秘的诊断价值并讨论该类疾病的声像图表现.方法 应用经会阴实时三维超声观察92例便秘患者张力期肛管、直肠的运动情况,测量直肠壶腹部至耻骨联合内下缘水平线的距离及膨出高度,测量静息期及张力期肛直角,对不同类型的后盆腔功能障碍性疾病进行诊断.结果 经会阴实时三维超声诊断61例后盆腔功能障碍性疾病患者,其中直肠膨出21例、肠疝4例、会阴体运动过度27例、盆底失弛缓综合征9例.结论 经会阴实时三维超声能够提供有价值的后盆腔解剖和功能信息,是一种能够有效识别后盆腔功能障碍所致便秘疾病分型的影像学检查技术.%Objective To evaluate the diagnostic value of transperineal real time three-dimensional ultrasound for the diagnosis of constipation caused by female posterior pelvic floor dysfunction,and to discuss the ultrasound appearance of this kind of disease.Methods Transperineal real time three-dimensional ultrasound was performed on 92 constipation patients to observe the position and mobility of anal canal and rectum.The retrovesical angle,rectocele depth and the rectal ampulla to the inferior symphyseal margin in horizontal distance were measured during rest and maximum Valsalva maneuver.Different types of posterior pelvic floor dysfunction disease were diagnosed according to the ultrasound findings.Results Translabial real time three-dimensional ultrasound diagnosed 61 cases of posterior pelvic floor dysfunction disease,including 21 cases rectocele,4 cases enterocele,27 cases perineal hypermobility and 9 cases pelvic floor achalasia syndrome.Conclusions Translabial real time three-dimensional ultrasound could provide useful information of posterior pelvic floor and is of great value in the diagnosis of female constipation caused by posterior pelvic floor dysfunction.
