复方透海散熏蒸治疗膝关节骨性关节炎的疗效观察及其对血清TNF-α、IL-6水平的影响

摘要

Objective:To observe the curative effect of compound fumigation in the treatment of knee osteoarthritis and to explore its mechanism.Methods:82 cases of knee osteoarthritis (KOA) were divided into control and observation groups at random,42 cases in the control group were treated with conventional therapy and rehabilitation guidance was given.The observation group (40 cases) was treated with compound Touhai San fumigation on the basis of the control group.The changes of serum TNF-α and IL-6 were observed by ELISA.Results:The total effective rate was 92.5％ in the observation group,which was significantly higher than that in the control group (73.8％),the difference was statistically significant (P ＜0.05).Compared with before treatment,the main symptom scores in the two groups were significantly decreased,the X-ray score of the knee was decreased and the HSS score was increased.The levels of serum TNF-ot and IL-6 were significantly decreased (P ＜ 0.05) The improvement of all above indexes in the observation group were more significant than those in control(P ＜ 0.05).Conclusion:Compound Touhai San fumigation can significantly improve the clinical curative effect of knee osteoarthritis patients,improve knee function,reduce serum inflammatory factors TNF-α,IL -6 levels.%目的:观察复方透海散熏蒸治疗膝骨性关节炎的临床疗效并探讨其作用机制.方法:将82例膝关节骨性关节炎(KOA)应用投币法随机分组.对照组42例予常规治疗并给予康复指导,观察组40例在对照组基础上联合中药复方透海散熏蒸治疗,4周为1个疗程.观察并记录2组患者治疗前后主要临床症状膝关节疼痛、晨僵、肿胀变化及膝关节X线评分、HSS评分的变化;EHSA法检测血清TNF-α、IL-6含量的变化.结果:观察组临床总有效率为92.5％,明显高于对照组之73.8％,差异具有统计学意义(P＜0.05).与治疗前比较,2组治疗后主要症状积分明显减少,膝关节X线评分降低,HSS评分升高;血清TNF-α、IL-6含量明显降低,差异均具有统计学意义(P＜0.05);观察组上述作用较对照组更明显(P＜0.05).结论:复方透海散熏蒸能够明显提高膝关节骨关节炎患者的临床疗效,改善膝关节功能,降低血清炎性因子TNF-α、IL-6水平时期部分作用机制.
