承气解毒汤辅助治疗急性重症胰腺炎的临床观察及其对CRP、PCT、PAF的影响

摘要

Objective:To observe the curative effect of severe acute pancreatitis treated with Chengqi Jiedu Tang and its effects on serum CRP,PCT and PAF.Methods:102 patients with severe acute pancreatitis were randomly divided into control group and observation group,51 cases in each one according to the visit sequence,with the random number table adopted.The routine treatment were received in the control group,and the observation group was treated with Chengqi Jiedu Tang on the basis of the control group.The clinical efficacy and adverse reactions of the 2 groups were observed after treating,and the levels of C reactive protein (CRP),procalcitonin (PCT) and platelet activating factor (PAF) were detected.Results:The total effective rate was 94.1％ in the observation group,which was better than that in the control group(76.5％) in 7 days of treatment.The times of blood amylase recovery,first ventilation,bowel sounds disappear,gastrointestinal decompression,abdominal pain relief in the observation group were significantly shorter than the control group after treatment;the levels of CRP,PCT and PAF in peripheral blood of the 2 groups patients after treatment were lower than those before treatment (P ＜ 0.05),the levels in the observation group were significantly lower than those in the control group (P ＜ 0.05);all patients had no obvious adverse reaction during the treatment.Conclusion:Chengqi Jiedu Tang treating severe acute pancreatitis (SAP) is safe and effective,which can significantly improve clinical symptoms and shorten the remission time of SAP patients with the main signs,decrease the levels of serum CRP,PCT,PAF.%目的:观察承气解毒汤辅助治疗急性重症胰腺炎(SAP)的临床疗效并探讨其作用机制.方法:102例急性重症胰腺炎患者随机分为对照组和观察组,对照组给予常规治疗,观察组在对照组基础上加以承气解毒汤治疗,2组均持续治疗7d.治疗结束后观察2组患者临床疗效及不良反应,治疗前后检测C反应蛋白(CRP)、降钙素原(PCT)、血小板活化因子(PAF).结果:观察组临床总有效率为94.1％,明显高于对照组的76.5％;治疗后,观察组患者的血淀粉酶恢复时间、首次通气时间、肠鸣音消失时间、胃肠减压时间、腹痛缓解时间均明显短于对照组(均P ＜0.05);治疗后2组患者外周血中CRP、PCT、PAF均较治疗前降低(均P＜0.05),观察组均显著低于对照组(均P＜0.05),2组患者用药期间均未见明显不良反应.结论:承气解毒汤辅助治疗急性重症胰腺炎安全有效,可明显改善SAP患者临床症状,缩短体征缓解时间,降低患者外周血中CRP、PCT、PAF.
