@@ 股骨头缺血性坏死(Legg-Calve'-Perthes'病)的病因尚未十分清楚,但皆公认其病理变化与股骨头骨骺供血障碍有关(1～2).在手术治疗上,方法很多,归纳起来,大致有下列三方面:(1)改变髋关节的负重线,如粗隆间旋转截骨术、粗隆下截骨术(3)、骨盆截骨术(4)等.%The treatment of 20 cases of Legg-Perthes' disease is described with a discussion.A11 the patients were treated with the procedure which entails synovectomy,drilling holes under the neck of the femur in capital femoral eplphysls with transplantation of the lateral femoral circumflex vessels into the capital femoral eplphysis.This procedure has the following advantages:(1) good blood supply contributes to the repair and growth of the epiphysis,(2) external circumflex branch of the vessels are thick and long so that their separation would not affect the blood supply to the surrounding tissues,(3) partial section of the synovium helps improve the pressure in the joint cavity and benefit the proliferation of the new blood vessels,in the transplant bundle,(4) the hole drilled in the neck of the femur wlll help enrich the blood supply of the epiphysis and the circulation by decreasing venous pressure of the bone.A total of 20 cases were so treated and 18 patients of this series were followed up over six months,83%(15/18) of which sbowed excellent or good results.
