应用胆总管囊肿切除带蒂回盲部肠段间置术治疗10例患儿,男3例,女7例,最小年龄2岁.手术切除囊肿后游离回盲部肠段,保留回盲动、静脉及其周围组织完整性,切取包括12～15cm回肠和4～5cm升结肠的肠段,顺时针旋转180°,使回肠与肝总管,盲肠与十二指肠或空肠吻合,最后将回肠与升结肠端端吻合.术后均顺利恢复.经随访无任何症状,钡餐检查均未见返流.并且讨论了手术的要点、注意事项和对手术的评价.%Choledochocystectomy and interposition of pedicled ileocecal loop designed by the authors were performed on 10 cases of congenital choledochal cyst after 1986.The youngest patient was 2 years old.The procedure includes:a total cholecystectomy,a resection of the ileocecal loop including ileum 12-15cm and acending colon 4-5cm with a pedicle keeping perfect blood supply and an interposition anastomosis between the common hepatic duct and duodenum or jejunum in its peristalsis direction.The donor intestine is rejoined by end to end anastomosis between ileum and ascending colon.All the 10 cases recovered from operation uneventfully.The follow-up results revealed symptoms free and reflux protected on barium meal in all cases.
