小儿闭合性肾损伤68例临床分析

摘要

自1979年1月至1991年12月共收治儿童闭合性肾损伤68例.5岁以下7例,5+～14岁61例.重型25例,轻型43例.肾切除7例,暂时性肾造瘘术4例,渗尿血肿引流术4例.认为肾外伤处理应及时正确判定肾脏损伤程度.根据临床表现结合B超和静脉尿路造影常可作出肾外伤程度综合判断.%From Jan.1979 to Dec.1991,68 children with blunt renal trauma were treated in Tainjing Children's Hospital.Seven cases were 3 mons to 4 yrs old and 61 cases between 5 yrs to 14 yrs of age.There were fortythree cases with minor injuries and 25 cases with major injuries.Fifteen cases were operated on and the operation were as follows:nephrectomy (7),nephrostomy (4),perirenal drainage(4).The nephrectomy rate was10.3% in the whole group (68 cases) but 20.8% in the group of major injuries (25 cases).Five cases had congenital renal malformations (horseshoe kidney 1;PUJ obstruction 4).The authors suggest that the management of renal trauma depends on an accurate determination of the extent of renal injury.The diagnosis and treatment of renal injuries are discussed in the text.
