经皮椎间孔镜两种手术入路治疗钙化型腰椎间盘突出症的疗效比较

摘要

目的:比较经皮椎间孔镜技术中两种不同手术入路治疗钙化型腰椎间盘突出症的疗效.方法:收集我院2013年10月到2015年2月之间行经皮椎间孔镜手术的67例钙化型椎间盘突出病人,均由同一术者完成手术,其中31例采用经椎板间入路(percutaneous endoscopic interlaminar discectomy,PEID),36例采用经椎间孔入路(percutaneous endoscopic transforaminal discectomy,PETD).每组病例均评估病人手术前后疼痛程度及直腿抬高角度,所得数据均经统计学处理.结果:经短期随访,两种入路对于病人疼痛症状的减轻均显著,无统计学差异.结论:两种入路对于钙化型椎间盘突出症的治疗均可行,但是经椎板间入路可以更充分切除钙化灶、分离神经根,进行充分减压.%Objective:To compare the effectiveness of two different approachs (interlaminar and transforainal approach) in the treatment of calcified lumbar disc herniation by percutaneous transforaminal endoscopic discectomy (PELD).Methods: Sixty-seven cases of calcified lumbar disc herniation treated by PELD from October 2013 to February 2015 were collected. All operations were completed by one operator. Thirty-one cases were treated with interlaminar approach (PEID). Thirty-six cases were treated with transforainal approach (PETD). The visual analogue scale (VAS) and the angle of straight-leg rising testing of the cases were recorded before and after the operations.Results:The short-term follow-up study showed the VAS of the cases treated by the two operative approaches was significantly decreased. There was no statistical difference between the two groups.Conclusion:It was efficient for patients to be treated by these two operative approaches. The posterior approach was better in calcifications removal, the nerve root separation and decompression.
