硅油填充后眼前段解剖参数改变的分析

摘要

目的 分析硅油填充术后有晶状体眼和无晶状体眼的眼前段解剖参数的改变.方法 采用回顾性研究方法,收集2017年9月至2017年12月行硅油填充术且随访≥3个月共55例(55眼)的临床资料,其中有晶状体眼35例(35眼)和无晶状体眼20例(20眼),用超声生物显微镜分别测量其术前和术后的眼前段解剖参数.结果 术后有晶状体眼的前房开放距离(AOD500)、小梁虹膜夹角(θ1)、小梁睫状体距离(TCPD)、虹膜睫状体距离(ICPD)均较术前减少(P＜0.05).术后有晶状体眼的虹膜晶状体接触距离(ILCD)、虹膜内表面与晶状体前表面的夹角(θ2)均较术前增加(P＜0.05).术后无晶状体眼的AOD500、θ1均较术前增加(P＜0.05).术后无晶状体眼的ICPD较术前减少(P＜0.05).结论 硅油填充术后有晶状体眼反映前房角开放程度的参数较术前减少,而无晶状体眼的相关参数增加.术后有晶状体眼较术前瞳孔 阻滞力增加、虹膜膨隆.术后有晶状体眼与无晶状体眼的虹膜睫状体距离较术前变短、巩膜睫状体夹角较术前变小.%Objective To analyse the anterior segment parameters of phakic eyes and aphakic eyes after silicone oil tamponade.Methods The data of 55 eyes of 55 cases with silicone oil tamponade and follow up≥3 months from Sep.2017 to Dec.2017 were collected and analyzed retrospectively.There were 35 phakic eyes and 20 aphakic eyes.The anterior segment parameters of all eyes were measured before and after surgery with ultrasound biomicroscopy.Results The angle opening distance (AOD500),trabecular-iris angle (θ1),trabecular ciliary process distance (TCPD) and iris-ciliary process distance (ICPD) of the phakic eyes after surgery were lower than those before surgery (P ＜ 0.05).The iris-lens contact distance (ILCD) and the angle between iris inner surface and anterior surface of lens (θ2) were increased after surgery compared with those before surgery (P ＜ 0.05).AOD500 and θ1 in aphakic eyes increased after surgery (P ＜ 0.05).ICPD in aphakic eyes decreased after surgery (P ＜ 0.05).Conclusion After silicone oil tamponade,the parameters related to the degree of opening of the ocular angle in phakic eyes are less than those before operation,while those of aphakic eyes increase.The pupillary block and iris distention in phakic eyes increase after silicone oil tamponade.The distance between iris and ciliary body of phakic eyes and aphakic eyes are shorter and the angle between sclera and ciliary body are smaller than those before surgery.
