曲安奈德前房注射在复杂白内障手术中的应用

摘要

Objective To investigate the effect and safety of intracameral injection of triamcinolone acetonide in complicated cataract surgery.Methods One hundred and thirty eight eyes of 138 cases with complicated cataract were divided into two groups:experimental group [n =39,(intracameral injection of triamcinolone acetonide during operation)] ; and control group [n =99,(peribulbar dexamethasone and systemic prednisone postoperatively)].The visual acuity,intraocular pressure,anterior chamber inflammation and other complications were compared between the two groups postoperatively.Results The visual acuities of experimental group were better than those of the control group in the early stage after the operation; but the difference was not statistically significant later.Anterior chamber reaction in experimental group was obviously better than that of the control group,and the difference was statistically significant.Postoperative intraocular pressure and other complications showed no significant difference between the two groups.Conclusion The intracameral injection of triamcinolone acetonide is safe and effective in complicated cataract surgery,which can reduce the early postoperative inflammatory reaction and promote early recovery of visual acuity.%目的 探讨在复杂白内障手术中前房内注射曲安奈德的效果和安全性.方法 138例(138只眼)复杂白内障手术分为两组:观察组(39例)术中前房内注射曲安奈德0.4～0.8 mg;对照组(99例)按常规术中术后球旁和全身应用地塞米松、泼尼松.比较两组视力、眼压、前房反应及并发症的发生情况.结果 术后视力观察组早期明显优于对照组,稳定后无差异;前房反应观察组较对照组轻,差异有统计学意义;而术后眼压变化及并发症两组比较差异无统计学意义.结论 复杂性白内障术中前房注射曲安奈德,可减轻早期术后炎症反应,促进视力早日恢复,提高了手术的安全性及效果.
