MRI增强扫描对脑内脱髓鞘假瘤的诊断价值

摘要

Objective To investigate the value of enhanced MRI on the diagnosis of intracerebral demyelination pseudotumor (DPT). Methods The plain and enhanced MR imaging were performed in 9 cases with DPT. All MR images were reviewed. Results Multiple lesions were found in 1 case, and solitary lesion was detected in the other 8 cases. The lesions showed hypointense on T1WT, and hyperintense on T2W1. On DWI, the lesions were mildly hypointense, isointense, or mildly hyperintense. Patchy enhancement was found in 2 cases, and linear enhancement perpendicular to lateral ventricles was found in the other 7 cases. Conclusions Enhanced MR imaging plays an important role, in the diagnosis of DPT. Linear enhancement perpendicular to lateral ventricles is the typical MR finding of intracerebral DPT.%目的 探讨MRI增强扫描对大脑脱髓鞘假瘤(demyelination pseudotumor,DPT)的诊断价值.方法 回顾性分析9例经病理或临床证实的大脑DPT患者MRI平扫及增强扫描的表现.结果 病灶多发1例,单发8例.MRI平扫显示,病变在TIWI上呈低信号,T2WI上呈高信号,DWI上呈略低、等或略高信号；增强扫描2例病灶内可见斑片状强化,余7例均见垂直于侧脑室的线条状强化.结论 MRI增强扫描对于DPT的诊断具有重要价值.垂直于侧脑室的线条状强化是大脑DPT的典型MRI表现.
