目的 初步探讨母乳成分微小RNA(microRNA,miR)与新生儿母乳性黄疸发生的相关性.方法 前瞻性选取2016年9 ~12月上海交通大学医学院附属上海儿童医学中心新生儿病房收治的母乳性黄疸所致重度高胆红素血症新生儿为观察组,同期在上海交通大学医学院附属仁济医院产科产后门诊随访的纯母乳喂养健康无黄疸新生儿为对照组,每组各4例,采用高通量测序的方法对两组母乳样本进行miR测序,筛选差异表达miR,应用生物信息学分析的方法预测靶基因.结果 生物信息学靶基因预测发现,母乳中存在大量与免疫调节和物质代谢相关的miR,如miR-148-3p、miR-30a-5p、miR-146-5p、let-7 f-5p、miR-181-5p、miR-22-3p、miR-182-5p;观察组母乳中miR-30a-5p、miR-146a-5p、miR-141-3p表达显著高于对照组,差异倍数分别为2.600、2.038、1.899倍,其中miR-141-3p靶基因包括葡萄糖醛酸基转移酶基因.结论 母乳miR对新生儿生长发育有多方面的影响,部分miR可能调控葡萄糖醛酸基转移酶基因表达导致新生儿母乳性黄疸的发生.%Objective To study the relationship of microRNA (miR) in breast milk and neonatal breast milk associated jaundice.Method From Sep.to Dec.2016,neonates with severe hyperbilirubinemia caused by breast milk were selected as the observation group,and breast-fed neonates without jaundice were selected as the control group.Their breast milk were collected,and the expression profile of miR in the breast milk was examined using miR sequencing.The variation of miR profile was screened using bioinformatics method,and miR related to neonatal breast milk associated jaundice was studied and the target genes were predicted.Result The breast milk contained many miRs associated with immunity regulation and metabolism,including miR-148-3p,miR-30a-5p,miR-146-5p,let-7f-5p,miR-181-5p,miR-22-3p,and miR-182-5p.The expressions of miR-30a-5p,miR-146a-5p and miR-141-3p in the observation group were significantly higher than the control group,and the differences were 2.600,2.038 and 1.899-fold,respectively.Uridine diphosphate glucuronosyl transferase1A1 gene was one of the target genes of miR-141-3p.Conclusion Breast milk miR may influence the growth and development,immunity regulation and metabolism of newborns.Some miRs,such as miR-141-3p,may be correlated with neonatal breast milk associated jaundice.
