重视老年健康管理应对老龄化挑战

摘要

“Population aging” is a worldwide tendency .China is one of the fastest aging countries in the world .It is a success of development but also presents a medical and social challenge .In this article, we discussed the updating concept , the implemention mode, and the major concerns of geriatric health services management .The geriatric health services management should be carried out in the following spirit: individual-oriented management , whole-process management , all-round management , all-aspect management , whole population health education , and multidisciplinary participation .The geriatric health services management should take advantages of“multidisciplinary health care mode” and“Internet Plus” platform.To promote an age-friendly health services system , the following points should be all taken into consideration: chronic diseases prevention , comprehensive geriatric assessment , early rehabilitation training, home health care, and palliative care services.%“人口老龄化”是当今全球人口学的变化趋势。自上世纪末以来，我国已步入老龄化社会，现已进入快速发展阶段，对老年健康管理提出严峻挑战，我国政府对此高度关注。本文从中国社会老龄化现状出发，阐述了老年健康管理应具有“六全”理念：全人管理、全程管理、全域管理、全方位管理、全科管理、及全民健康教育；论述了老年健康管理应采用多学科团队照护模式及“互联网＋”平台的新型管理模式；强调了老年健康管理的重点应为慢病预防、综合评估、早期康复、居家照护、及安宁疗护。
