血浆致动脉硬化指数与2型糖尿病患者大血管病变的相关性分析

         

摘要

Objective To explore proper clinical index in predication of risk for macroangiopathy in type 2 diabetes mellitus (T2DM) patients. Methods A total of 110 type 2 diabetes mellitus patients were divided by area of vasculopathy into macroangiopathy group (group A) with 70 cases and non-macroangiopathy group (group B) with 40 cases. Another 40 healthy people in the same period were chosen as control group. Their blood pressure and blood lipid were detected, and their body mass index (BMI) and atherogenic index of plasma (AIP) were calculated. Correlation analysis was made on vasculopathy status in groups A and B. Results Compared with the control group, the T2DM group had increased glycosylated hemoglobin (HbA1c), systolic blood pressure (SBP), diastolic blood pressure (DBP), total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), AIP, and decreased high-density lipoprotein cholesterol (HDL-C), and their differences all had statistical significance (P<0.001 or P<0.05). Compared with group B, group A had statistically significant differences of duration of diabetes mellitus, HbA1c, SBP, DBP, TC, TG, LDL-C, HDL-C and AIP (P<0.001 or P<0.05);while group B had much lower incidence of microangiopathy than group A (P<0.05). Conclusion AIP can provide accurate and convenient risk of macroangiopathy in type 2 diabetes mellitus patients. Accuracy of diagnosis can be remarkably improved by combination of blood lipid analysis results.%目的:找到合适临床指标以预测2型糖尿病(T2DM)患者大血管病变发生的危险性。方法110例2型糖尿病患者,根据血管病变发生范围太小分为合并大血管病变组(A组)70例和无大血管病变组(B组)40例,同时选取同期40例健康志愿者作为对照组;测量各自的血压、血脂,同时计算体质量指数(BMI)和血浆致动脉硬化指数(AIP),结合A、B两组血管病变情况做相关性分析。结果与对照组比较, T2DM组呈现出糖化血红蛋白(HbA1c)、收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)升高,高密度脂蛋白胆固醇(HDL-C)降低, AIP值升高,差异均具有统计学意义(P<0.001或P<0.05);A组糖尿病病程、HbA1c、SBP、DBP、TC、TG、LDL-C、HDL-C、AIP与B组相比差异均具有统计学意义(P<0.001或P<0.05);B组的微血管病变发生率显著低于A组(P<0.05)。结论 AIP值能准确方便地预测2型糖尿病患者大血管病变发生的危险性,结合血脂的分析结果能大大提高诊断准确性。
