Encor真空负压旋切系统在乳腺良性肿物切除术中的应用

         

摘要

目的 探讨超声引导下Encor真空负压旋切系统对乳腺良性肿物切除术中的应用价值. 方法 2010年7月~2011年7月采用超声定位引导,利用Encor真空负压旋切系统(美国强生公司Model CART01型)对119例391个乳腺良性肿物进行切除,术后加压包扎48 h.切除标本常规行快速石蜡病理检查. 结果 117例389个乳腺良性肿物被完全切除,1例肿物少量残留,1例手术失败改传统开放性手术.术后病理诊断:乳腺纤维腺瘤276个,纤维腺瘤伴导管内乳头状瘤25个,乳腺导管扩张性症68个,导管内乳头状瘤14个,上皮非典型增生6个,乳腺腺病2个.术中大量出血(10~30 ml)6例,术后血肿3例,皮下瘀斑11例.119例术后随访3~15个月,平均8个月,血肿、瘀斑消失,乳房外形正常,1例残余肿瘤无明显变化,余118例未发现病灶残留、复发. 结论 应用超声引导Encor真空负压旋切系统切除乳腺良性肿物效果好,微创优势明显,操作简单、安全,对乳腺良性肿物可完全切除,值得推广.%Objective To evaluate the value of ultrasonography-guided Encor breast biopsy system for benign breast lesions. Methods Since July 2010 to July 2011, under the guidance by ultrasonography, we removed 391 benign breast lesions from 119 patients by using Encor breast biopsy system. Compression hemostasis for 48 hours were done in all the cases. Fast paraffin-embedded sections were made for pathological examination after the operation. Results Complete resection was achieved in 117 patients (389 lesions ) ; in the other 2 patients, one had residual tumor tissues, and the other were converted to traditional open surgery. Postoperative pathological examination revealed fibroadenoma in 276 lesions, fibroadenoma complicated with papilloma in 25, mammary ductal ectasia in 68, intraductal papilloma in 14, atypical ductal hyperplasia in 6, and breast adenosis in 2. Six patients had massive hemorrhage (10-30 ml) during the operation, 3 patients developed postoperative hematoma, and 11 patients showed subcutaneous ecchymosis. The patients were followed up for 3 to 15 months with a mean of 8 months, during which, the hematoma and ecchymosis were cured, the appearance of the breast was normal in all the cases. The patient with residual tumor tissues showed no significant changes; no recurrence or residual tumor tissues were detected in the other 118 patients. Conclusion Ultrasonography-guided Encor breast biopsy system is a minimally invasive, safe, and simple method for complete resection of benign breast tumor.
