股骨头坏死与类风湿性关节炎患者人工全髋关节置换疗效分析

摘要

Objective To evaluate the functionaloutcome of total hip arthroplasty in patients with Osteonecorsis of femoral head and rheumatoid arthritis. Method We retrospectively analyzed the patients who underwent total hip arthroplasty in our hospital between September 2003 and September 2006(332 patients / 412 hips). The functional outcome was measuredon the basis of the Hams score and the Western Ontario and McMaster Universities Arthritis Index(WOMAC). Result 274 patients (307 hips) were followed up. The follow-up duration ranged from 3 ~6. 2 years ( mean 4. 3 years) , and the follow-up rate was 74. 5% . The X-Ray showed no obvious radiolucent lines, varus or valgus displacement, and aseptic loosening during the Follow-Up period. The mean Harris score increased from 44. 7 before operation to 91. 5 after operation in ONFH patients and from 33. 5 to 89. 7 in rheumatoid patients respectively. The mean WOMAC score decreased from 117. 8 to 73. 5 in ONFH patients and from 109. 3 points to 70. 8 points in Rheumatoid patients respectively. Conclusion The functional outcome of primary THAs in ONFH and RA patients are excellent. And there was no statistically significant difference between the two groups.%目的 评价股骨头坏死与类风湿性关节炎患者人工全髋关节置换治疗效果.方法 回顾性分析本院2003年9月至2006年9月应用非骨水泥解剖型人工髋关节假体治疗股骨头坏死与类风湿性关节炎患者332例,共412髋.采用Harris评分及WOMAC评分评价患者功能康复情况.结果 总计274例患者,307髋获得随访,随访时间3～6.2年,随访率74.5%.随访期间X线检查未见明显透光线、假体内外翻以及股骨柄假体无松动并发症.股骨头坏死患者Harris评分,术前平均44.7分,术后平均91.5分; WOMAC评分术前平均117.8分,术后平均73.5分.类风湿性关节炎患者术前Harris评分33.5分,术后平均89.7分; WOMAC评分,术前平均109.3分,术后70.8分.结论 股骨头坏死患者与类风湿性关节炎患者人工关节置换术后,两组患者疗效差异无显著性.
