目的:探讨静脉注射大剂量免疫球蛋白在老年患者长期机械通气肺部感染治疗中的作用.方法:78例因呼吸衰竭行长期机械通气后并发肺部感染的老年患者随机分为观察组和对照组,对照组患者按常规疗法,观察组在常规治疗的基础上给予大剂量免疫球蛋白辅助治疗.结果:观察组的治疗有效率明显高于对照组,肺部感染再发率、重症肺部感染发生率和患者死亡率明显低于对照组,差异有统计学意义(P<0.05);两组患者治疗后的血清免疫指标较治疗前均有所改善,观察组患者治疗后血清免疫指标改善情况较对照组明显,差异有统计学意义(P<0.05).结论:因长期机械通气后并发肺部感染的老年患者应用大剂量免疫球蛋白辅助治疗临床效果满意,同时可预防再发肺部感染,降低感染死亡率.%Objective: To investigate the effect of large dose of intravenous immunoglobulin in the treatment of elderly patients with prolonged mechanical ventilation and pulmonary infection.Methods:78 elderly patients with pulmonary infection were randomly divided into observation group and control group. The control group was treated with conventional therapy, and the observation group was treated with large dose of immunoglobulin.Results:The effective rate of the observation group was significantly higher than that of the control group, the incidence rate of pulmonary infection, severe pulmonary infection and mortality were significantly lower than the control group, the difference was statistically significant (P<0.05); the serum level of immune index in the two groups were improved compared with that in the observation group,and the difference was statistically significant (P<0.05).Conclusion:The clinical effect of large dose of immune globulin in elderly patients with pulmonary infection after prolonged mechanical ventilation is satisfactory, and it can prevent the recurrence of pulmonary infection and reduce the mortality.
