中心螺旋CT表现特征与高场强MRI诊断中心型肺癌关系探讨

摘要

目的：通过试验探究螺旋CT表现特征与高场强MRI诊断中心型肺癌关系。方法：选取2012年6月～2014年6月期间在我院胸外科行行纤支镜活检或手术切除并经病理证实的原发性中心型肺癌的患者100例，均行螺旋CT及高场强MRI检查，比较两种检查方法的检查结论及联合两种检查方法后的诊断结论，探究中心型肺癌螺旋CT表现特征与高场强MRI诊断中心型肺癌关系。结果：螺旋CT与高场强MRI联合诊断的准确率（93%）高于单独螺旋CT诊断中心型肺癌的准确率（83%），误诊率明显降低，均有P＜0.05，差异有统计学意义。相比于单独螺旋CT诊断（8%），螺旋CT联合高场强MRI诊断中心型肺癌的误诊率（1%）明显降低，P＜0.05，差异有统计学意义。螺旋CT诊断中心型肺癌的误诊病例主要发生于隐性癌病例及Ⅰ期中心型肺癌的病例中。结论：螺旋CT表现特征是有一定的诊断意义的，可以准确诊断大部分中心型肺癌，当与高场强MRI检查结果相结合时，其诊断的准确性大大提高，尤其对于隐性癌的诊断，更需要螺旋CT与高场强MRI的联合检查，才能获得一定的诊断准确率。%Objective: To explore the relationship of the spiral CT performance characteristics and high field strength MRI in the diagnosis of central type lung cancer.Methods:Selected 100 cases of primary central type lung cancer patients from June 2012 to June 2014 in thoracic surgery of our hospital who were conducted bronchoscope biopsy or surgical removed and Confirmed by pathology.Conducted spiral CT and high field MRI examination,Compared examination conclusion of two inspection methods and diagnosis conclusion of uniting two methods,researched the relationship between spiral CT performance characteristics of central type lung cancer and high field MRI diagnosis of central type lung cancer.Results:The accuracy rate of spiral CT and high field MRI combined diagnostic(93%)increased obviously in diagnosis of central type lung cancer by comparing to the single spiral CT(83%),the misdiagnosis rate decreased significantly,bothP<0.05,the difference was statistically significant.Compared to the single spiral CT diagnosis(8%),the misdiagnosis rate(1%) of spiral CT combined with high field MRI in the diagnosis of central type lung cancer obviously decreased,P<0.05,the difference was statistically significant.The misdiagnosis of spiral CT in the diagnosis central type lung cancer occurred mainly in the recessive cancer cases and I stage central type lung cancer cases.Conclusion:Spiral CT performance characteristics have certain sense to the diagnosis; can accurately diagnose the majority of central type lung cancer. When combining with the findings of MRI high field strength,the diagnosis accuracy is greatly improved.Especially for occult cancer diagnosis,it is more needed combining with spiral CT and high field MRI examination to obtain certain accuracy rate of diagnosis.
