老年糖尿病患者心理干预分析及对血糖稳定性的影响

摘要

Objective:To explore the psychological intervention on elderly diabetes mellitus (DM) analysis of the psychological state of patients and the effects on blood glucose stability.Methods:86 cases of elderly patients with type 2 DM,according to single blind randomized control method is divided into observation group and control group,43 cases in each.The observation group received routine nursing and psychological intervention,while the control group only received routine nursing care.Negative emotions,the level of resilience and survival of the two groups of patients was assessed before and after the intervention;the dynamic glucose monitoring system (CGMS) detection of two groups of patients with blood glucose level stability index.Results:Before the intervention,the two groups had no statistical significance in patients with negative emotions,psychological resilience and quality of life,the stability of blood glucose difference (P ＞ 0.05).After the intervention,the observation group HAMD score,GAI score was significantly lower than the control group,CD-RISC score,GQOL score was significantly higher than the control group (P ＜0.05);the observation group blood glucose fluctuations (MAGE),daytime blood glucose mean absolute difference (MODD),standard deviation of blood glucose (SDBG) level was significantly lower than the control group (P ＜ 0.05).Conclusion:Psychological intervention can optimize the elderly DM patients with mental state,enhance mental flexibility and enhance the quality of life,with the stability of blood glucose level in patients with Yu Weichi's help.%目的:探讨心理干预后老年糖尿病(DM)患者心理状态分析及对血糖稳定性的影响.方法:老年2型DM患者86例,按单盲随机对照法分为观察组、对照组,各43例.观察组接受常规护理联合心理干预,对照组仅接受常规护理.评估干预前后两组患者的负面情绪、心理弹性水平及生存状态;采用动态血糖监测系统(CGMS)检测两组患者的血糖稳定性指标水平.结果:干预前,两组患者的负面情绪、心理弹性及生活质量、血糖稳定性差异无统计学意义(P＞0.05).干预后,观察组的HAMD评分、GAI评分值明显低于对照组,CD-RISC评分、GQOL评分值明显高于对照组(P＜0.05);观察组血糖波动幅度(MAGE)、日间血糖平均绝对差(MODD)、血糖水平标准差(SDBG)水平明显低于对照组(P＜0.05).结论:心理干预可优化老年DM患者的心理状态,增强心理弹性并提升生活质量,有助于维持患者的血糖稳定性.
