实时三平面缩流宽度法半定量瓣膜反流影响因素的体外模型研究

摘要

Objective To evaluate the accuracy and influence factors during semi- quantization of valve regurgitation by the vena contracta width with the real-time triplane echocardiography. Methods The vena contracta width of the regurgitant orifice with the diameter of 2 mm, 4 mm, 6 mm, 8 mm were observed and measured in the condition of different pressure gradient and the value with the real-time tri-plane echocardiography using the self-made valve regurgitation model was obtained. Results The vena contracta width did not vary with the pressure gradient and the color Doppler surveying range ( P = 0. 06 ,P = 0. 59 ). But it was positively correlated with the added value ( P = 0. 00 ). And the vena contracta widths measured by real-time tri-plane echocardiography were wider than the theoretic vena contracta width and the diameter ofthe real regurgitant orifice ( P = 0. 00 ) when an added value of more than - 15 db was obtained. The width gain of vena contracta ( - 20 db ) was similar to the diameter of the real regurgitant orifice ( P = 0. 74 ) but a little bit wider than the width of theoretic vena contracta ( P = 0. 00 ). Conclusion The width of vena contracta measured by real-time tri-plane echocardiography was influenced by the added value. Under the low gain value the width of vena contracta measured are similar to the diameter of the real regurgitant orifice.%目的 探讨实时三平面条件下利用缩流宽度半定量瓣膜反流的准确性及其影响因素. 方法 利用自制的体外瓣膜反流模型,反流孔口直径为2 mm、4 mm、6 mm、8 mm.采用三维超声心动图三平面方法,在不同压差、不同增益和不同彩色多普勒速度测定范围下测量反流缩流宽度,并与实际孔口直径及理论缩流宽度相比较.结果 缩流宽度不随压差和彩色多普勒速度测定范围的改变而改变(P=0.06,P=0.59);但受增益条件的影响,缩流宽度测值随增益的增加而增加(P=0.00),增益≥-15 db时实测值大于理论缩流宽度和实际孔口直径(P=0.00,P=0.00),在增益为-20 db条件下,实测值较接近孔口直径(P=0.74),但略大于理论缩流宽度(P=0.00).结论 在实时三平面条件下,缩流宽度测值受增益影响,采用较小增益时缩流宽度测值较接近实际孔口直径.
