原发性高血压患者肾脏血流动力学变化的CT灌注研究

         

摘要

Purpose To investigate the value of multi-slice CT (MSCT) perfusion in monitoring the status of renal function in patients with primary hypertension. Materials and Methods Thirty patients diagnosed as primary hypertension were selected as the patient group, and thirty healthy persons who met the study criteria were selected as the control group. For both groups, dual-kidney CT perfusion was conducted to obtain blood flow (BF), blood volume (BV), mean transit time (MTT) of contrast agent, time to peak (TTP). The difference between patient group and control group was compared, followed by an analysis of the correlation between the measured perfusion parameters and the glomerular filtration rate (GFR) measured using renal dynamic imaging with 99Tcm-DTPA. Results Compared with the control group, the patient group demonstrated a reduced BF value (F=28.093, P<0.01) and MTT (F=13.066, P<0.05), and a lengthened TTP (F=4.068, P<0.05) in the renal cortex, and an elevated MTT in the renal medulla (F=5.066, P<0.05). In renal cortex, the measured GFR was found positively correlated with the measured BF (r=0.433, P<0.01) and negatively correlated with TTP and MTT (r=-0.351,-0.423, P<0.05). No significant correlation was found between the BV in renal cortex and all the measured perfusion parameters in medulla (P>0.05). Conclusion MSCT perfusion can be used to observe the changes of kidney microcirculation and to assess renal function in patients with hypertension, which provides direct evidence for the diagnosis of hypertensive kidney damage.%  目的探讨多层螺旋CT(MSCT)灌注成像监测原发性高血压患者肾脏功能状况的临床应用价值。资料与方法选取临床明确诊断的原发性高血压患者30例为病例组,30例符合研究标准的正常人为对照组,行双肾CT灌注扫描,获得肾脏灌注图和灌注参数肾血流量(BF)、肾血容量(BV)、肾脏对比剂平均通过时间(MTT)和肾脏强化CT值上升时间(TTP),比较两组灌注参数差异,并分析99Tcm-DTPA肾动态显像获得的肾小球滤过率(GFR)与灌注参数的相关性。结果病例组肾皮质BF值较对照组减小(F=28.093, P<0.01),MTT(F=13.066, P<0.05)及TTP(F=4.068, P<0.05)延长;肾髓质MTT值较对照组延长(F=5.066, P<0.05)。肾皮质BF值与GFR呈正相关(r=0.433, P<0.01);TTP、MTT值与GFR呈负相关(r=-0.351、-0.423, P<0.05)。肾皮质BV值与髓质各灌注参数均无显著相关性(P>0.05)。结论 MSCT肾灌注成像可用于观察高血压患者肾脏微循环的改变,反映肾功能,为临床诊断高血压肾损害提供依据。
