16层螺旋 CT 肺容积与肺密度指标与肺通气功能的相关性研究

摘要

目的：通过分析16层螺旋 CT （MSCT）肺容积和肺密度指标与肺通气功能指标的相关性，探讨 MSCT 与肺功能试验（PFT）指标的关系。方法选取64例受试者行胸部 MSCT 及 PFT 检查。MSCT 观察不同呼吸时相下肺密度指标：深吸气末平均肺密度（MLDin）、深呼气末平均肺密度（MLDex）和肺密度差（MLDex-MLDin），PFT 观察肺功能指标包括肺总量（TLC）、残气量（RV）、残气量与肺总量比（RV/TLC）、用力肺活量（FVC）、第1秒用力呼气容积（FEV1）和第1秒用力呼气容积与用力肺活量比（FEV1/FVC）。结果64名受试者依据 CT 肺气肿诊断标准，可分为MSCT 肺气肿组40例和 MSCT 正常组24例，依据 PFT 肺气肿诊断标准，可分为 PFT 肺气肿组38例和 PFT 正常组26例。MSCT 肺容积指标与肺功能指标均呈不同程度的正相关（P ＜0．05），绝对指标 Vin、Vex 与 PFT 静态肺容积指标 TLC、RV 的相关性高于肺通气功能指标 FVC、FEV1。MSCT 肺密度指标与 PFT 静态肺容积指标呈负相关（P ＜0．05），与肺通气功能指标呈正相关（P ＜0．05）。结论MSCT 肺容积和肺密度指标与 PFT 肺通气功能指标有较好的相关性。MSCT 检测能较好地判断患者肺功能状况，具有较好的临床应用的价值。%Objective To analyze 16 slice spiral CT (MSCT)associated lung volume,density index and the index of pulmonary ventilation function,discusses MSCT and pulmonary function test (PFT)index relationship.Methods 64 subjects underwent chest MSCT scan and PFT.Lung density index under different respiratory phase MSCT study:deep inspiratory mean lung density (MLDin),deep end expiratory lung density (MLDex)and mean lung density difference (MLDex-MLDin).PFT observation of pulmonary function indexes including total lung capacity (TLC),residual volume (RV),residual volume and total lung capacity ratio (RV/TLC),forced vital capacity (FVC),forced expiratory volume in one second (FEV1)and forced expiratory volume in one second and forced vital capacity ratio (FEV1/FVC).Results On the basis of CT emphysema diagnosis standard,64 subjects can be divided into MSCT emphysema group 40 cases and MSCT 24 cases of normal group.On the basis of PFT emphysema diagnosis standard,they can be divided into PFT emphysema group 38 cases and PFT normal group26 cases.A different degree of positive correlation of MSCT lung vol-ume index and pulmonary function indices(P <0.05),correlation between Vin,Vex and PFT absolute index of static lung volume index of TLC,RV higher than the pulmonary function indices FVC,FEV1.MSCT was negatively related to lung density index(P <0.05)and PFT static lung volume index,were positively correlated with the pulmonary func-tion indices(P < 0.05).Conclusion MSCT lung volume and density index and PFT index of pulmonary ventilation function have good correlation.MSCT detection can better judge of pulmonary function in patients,with good clinical application value.
