血清 RF、抗 CCP 抗体和抗 MCV 抗体水平对早期类风湿关节炎的诊断价值

摘要

Objective To explore the diagnostic value of Anti-Mutated citrullinated vimentin antibodies to early rheumatoid arthritis.Methods Chose 50 cases early RA patients from our hospital in May 2012-April 2014 set for early RA group,chose the same period in our hospital of 30 cases no RA patients set for the no RA group,chose 30 healthy-check-up people as healthy controls in the same period in our hospital.Testing all subjects rheumatoid factor (RF),ser-um anti ring citrulline peptide (CCP)antibody,fight the MCV antibody levels,contrast analysis of three kinds of test indexes sensitivity and specificity in the diagnosis of early RA,analysis the correlation of the three.Results The levels of Anti-MCV antibodies,Anti-CCP antibodies and RF in Early RA group and the no RA group were significantly higher than that of healthy controls,he early RA group were significantly higher than that of higher than the RA group,the difference has statistical significance (P <0.05);The positive rate of Anti-MCV antibodies,Anti-CCP antibodies and RF in Early RA group were higher than in the RA group and healthy controls,the early RA group had a significantly higher rate of positive of antibody of MCV resistance RF,anti CCP antibody positive rate,differences were statistically significant (P <0.05);Resist the MCV antibodies in the diagnosis of early RA sensitivity and specificity were 82.00%and 92.00% respectively,significantly higher than that of the RF and the sensitivity and specificity of CCP antibody, the difference was statistically significant (P <0.05);Pearson correlation analysis showed that the MCV antibody re-sistance and anti CCP antibody and RF were significant positive correlation (P <0.05).Conclusion Anti-MCV anti-bodies,Anti-CCP antibodies and RF in patients with early RA are high serum expression,the sensitivity and specificity of Anti-MCV antibodies to the diagnosis of early RA is high,and can be used as auxiliary diagnosis of early RA.%目的：探讨抗突变型瓜氨酸波形蛋白（MCV）抗体对早期类风湿关节炎（RA）的诊断价值。方法选择2012年5月-2014年4月我院收治的早期 RA 患者50例为早期 RA 组，选择同期于我院就诊的非 RA 患者30例为非RA 组，另选择同期于我院健康查体者30例作为对照组。检测所有受试者血清类风湿性因子（RF）、抗环瓜氨酸肽（CCP）抗体和抗 MCV 抗体水平，对比分析三种检测指标诊断早期 RA 的敏感性和特异性，分析三者的相关性。结果早期 RA 组和非 RA 组血清 RF、抗 CCP 抗体、抗 MCV 抗体水平均显著高于对照组，早期 RA 组血清 RF、抗 CCP抗体和抗 MCV 抗体水平高于非 RA 组，差异均有统计学意义（P ＜0．05）；早期 RA 组 RF、抗 CCP 抗体和抗 MCV 抗体阳性率均高于非 RA 组和对照组，早期 RA 组抗 MCV 抗体阳性率显著高于 RF、抗 CCP 抗体阳性率，差异均有统计学意义（P ＜0．05）；抗 MCV 抗体诊断早期 RA 的敏感性和特异性分别为82．00％和92．00％，显著高于 RF 和抗 CCP抗体的敏感性和特异性，差异有统计学意义（P ＜0．05）。Pearson 相关分析显示，抗 MCV 抗体与抗 CCP 抗体、RF 均呈显著正相关（P ＜0．05）。结论抗 MCV 抗体、抗 CCP 抗体和 RF 在早期 RA 患者血清中高表达，抗 MCV 抗体诊断早期 RA 的敏感性和特异性均较高，可以作为早期 RA 的辅助诊断方法。
