IgA肾病扁桃体归巢受体L-选择素、VLA-4α、LFA-1和血管地址素ICAM-1、VCAM-1表达及意义

摘要

目的:探讨L-选择素、VLA-4α、LFA-1、ICAM-1、VCAM-1在IgA肾病 (IgA nephropathy,IgAN)患者与非肾炎慢性扁桃体炎患者扁桃体的表达及意义.方法:收集10例伴有慢性扁桃体炎的IgAN患者扁桃体(IgAN组),这些患者经肾活检诊断为IgAN,10例不伴IgAN慢性扁桃体炎(Chronic tonsillitis,CT组)作为对照.应用Western blot技术定量分析两组扁桃体组织L-选择素,VLA-4α、LFA-1、ICAM-1、VCAM-1的表达,同时采用免疫组织化学技术检测扁桃体L-选择素、VLA-4α、LFA-1表达,观察ICAM-1,VCAM-1在生发中心和滤泡间区高内皮小静脉(High endothelial venules HEV)分布.结果:Western blot技术显示IgAN组及CT组扁桃体L-选择素、VLA-4α、LFA-1、ICAM-1、VCAM-1的表达分别为[(1.10±0.09;0.80±0.14)、(0.39±0.05;0.24±0.04)、(0.41±0.06;0.29±0.06)、(1.36±0.04;1.18±0.09)、(2.56±0.63;1.60±0.32)].免疫组织化学检测两组各归巢受体表达差别有显著统计学意义,IgAN扁桃体生发中心和滤泡外区HEV内血管地址素表达增强.结论:扁桃体淋巴细胞归巢异常是伴有慢性扁桃体炎的IgAN患者的重要特征,IgAN的发病机制与之有密切关联.%Ovjective:To investigate the expression of ;L-seledctin,VLA-α,LFA-1,ICAM-1,VCAM-1 in the tonsil of patients with IgA nephropathy(IgAN).Methods:Sthdied the tonsil samples of 10 patients with biopsy-proven IgAN (IgAN grop),(6male,mesn age 24±9)and 10 adylts who had chronic tonsiltis without the history of kidney disease(CT group),(5 males,mean age 27±12).Wetern blot tedhnique was used to analyze L-selectin,VLA-4α,LAF-1,ICAM-1,VCAM-1 expression in the tonsil.Also,using immunogistichem istry(SABC),we observed the expression of homing receptors L-selectin,VLA-4α,LFA-1 in the tonsils.In geminal centers and high enothelial venules(HEV),vasular addressin(ICAM-1,VCAM-1)was examined at the mean time.All data was fed into a personal computer using SPSS13.0 statistics soft ware:Results:Western blot showed the statistically enganced expression of L-seledctin,VLA-4α,LFA-1,ICAM-1and VCAM-1 in IGAN group compared with CT group(1.10±0.09vs0.80±0.14),(0.39±0.05vs0.24±0.04),(0.41±0.06vs0.29±0.06).(1.36±0.04vs1.18±0.09),(2.56±0.63vs1.60±0.32),respectively.On immunohistochem ical analysis,changes in goming redcetors of the tonsil were statically significance,and there was the enganced expression of the vascular addressins in the tonsilar gern inal centers and HEV in IgAN group.Conculsion:While suffering from cgronic tonsilitis,the patients with IgAN is characteristic of the increased expresson of L-selectin,VLA-4α,LFA-1,VCAM-1 in the tonsil.The pathogenesis of IgAN is closely realted to lymphocyte abnom ally homing to the tonsil.
