Objective To observe the clinical curative effect of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) combined with sputum drainage by beating the back and with positive pressure vibration. Methods Sixty patients with AECOPD who were treated in Zhangjiagang First People's Hospital from September 2016 to May 2017 were randomly divided into observation group (n=30) and control group (n=30). On the basis of routine treatment, the control group was treated with the method of expelling the sputum, the observation group on the basis of the control group combined with Acapella Duet vibration expectoration auxiliary sputum. Observation of two groups of patients with expectoration and symptoms improved. Results (1)The average 24 hours sputum volume of observation group is more than that in the control group, sputum properties and better color days shorter than that for the control group; (2)The change of blood gas analysis in carbon dioxide partial pressure (PCO2) after the treatment in the observation group was better than that in the control group and the hospitalization time was shorter than that of the control group; (3)The efficiency of prognosis of the patients in the observation group was 90%, exceeding 70% of the control group. Conclusion Beating back combined with positive pressure vibration expectoration can effectively promote the excretion of sputum, improve the ventilation function, shorten the hospitalization time, and contribute to the prognosis of patients with AECOPD.%目的 观察叩背排痰联合正压振动排痰法对于慢性阻塞性肺疾病急性发作(AECOPD)患者治疗的临床疗效.方法 选取张家港市第一人民医院2016年9月—2017年5月就诊的60例AECOPD患者作为研究对象,随机分为观察组和对照组,每组各30例.在给予常规治疗的基础上,对照组予以叩背法排痰,观察组在对照组基础上联合Acapella Duet振动排痰器辅助排痰.观察两组患者排痰效果及症状改善情况.结果 (1)观察组平均24 h排痰量多于对照组,痰液性质及颜色好转天数较对照组缩短;(2)观察组治疗前后血气分析中二氧化碳分压(PCO2)改善较对照组明显,住院天数较对照组缩短;(3)观察组患者预后有效率为90%,对照组预后有效率为70%,观察组高于对照组.结论 叩背联合正压振动排痰能有效促进AECOPD患者痰液排出,改善通气功能,缩短住院时间,有助于预后.
