改良肘关节前人路微型钢板内固定治疗尺骨冠状突骨折

摘要

Objective To investigate the curative effects of mini-plate internal fixation through modified anterior approach of elbow joint for the treatment of ulna coronoid process fracture.Methods From January 2013 to December 2015,11 cases of ulna coronoid process fracture were treated by mini-plate internal fixation through modified anterior approach of elbow joint.The functional recovery of elbow joint was evaluated after the operation.Results All the patients were follow-up for 8 to 25 months with an average of 15.4 months.Bone healing was achieved in all the cases showed by X-ray.The range of motion was (115.00±8.56)° for elbow flexion and extension,(130.00±6.58)° for forearm rotation.According to Mayo Elbow Performance Score,the elbow function was rated as excellent in 6 cases,good in 4 cases,fair in 1 case.Mild heterotopic ossification occurred in 1 case.Lateral brachial cutaneous nerve injury occurred in 1 case.No infection,vascular injury,joint dislocation,traumatic arthritis and stiffness occurred.Conclusion In the treatment of ulna coronoid process fracture,elbow joint can be fully exposed by modified anterior approach and mini-plate internal fixation is reliable to achieve good clinical effect.%目的 探讨改良肘关节前入路微型钢板治疗尺骨冠状突骨折的疗效.方法 自2013年1月至2015年12月,对1 1例尺骨冠状突骨折患者行改良肘关节前人路切开复位微型钢板内固定,观察患者肘关节功能恢复情况.结果 术后11例均获得随访,时间8~25个月,平均15.4个月.X线片示所有患者骨折愈合.肘关节屈伸活动范围(115.00±8.56)°,前臂旋转活动度为(130.00±6.58)°,Mayo肘关节功能评分:优6例,良4例,一般1例.1例轻度异位骨化,1例臂外侧皮神经损伤,无感染、血管损伤、关节脱位、创伤性关节炎及僵硬发生.结论 对于尺骨冠状突骨折,采用改良肘关节前入路暴露充分,微型钢板内固定牢固可靠,临床效果良好.
