彩色多普勒超声检查法对药物流产不全后宫内残留物的诊断价值

摘要

目的分析彩色多普勒超声检查法（color Doppler ultrasonography，CDS）对药物流产不全后宫内残留物的诊断价值。方法选择2014年3月至2015年4月本院收治的药物流产不全患者72例为研究对象，所有患者均先行CDS检查，观察患者子宫二维声像图及血流特点，后行尿人绒毛膜促性腺素（human chorionic gonadotropin，HCG）检测。检查完毕后，所有患者均行清宫处理，所获组织标本送病理检查。比较上述两种检查方法对药物流产不全后宫内残留物诊断的准确率、灵敏度及特异度。结果72例患者中，67例（93.06%）可检测到血流信号；脉冲多普勒呈低阻型动脉血流频谱，阻力指数（resistance index，RI）为0.36～0.55；且CDS对宫内残留物诊断的准确率、灵敏度及特异度均高于HCG检测（P＜0.05）。结论 CDS在药物流产不全后宫内残留物的诊断中具有较高价值，能够为临床后续治疗提供可靠的诊断依据。%Objective To analyze the diagnostic value of color Doppler ultrasonography (CDS) in the diagnosis of incomplete medical abortion. Method 72 cases of patients with incomplete medical abortion in our hospital from March 2014 to April 2015 were selected as the research objects. Peformed CDU examination first, and then detected the urine level of human chorionic gonadotropin (HCG). After the evacuation of uterus, took pathological examination on tissues acquired. Took pathological results as controls, compared the sensitivity and specificity of two above mentioned methods to the diagnosis of incomplete medical abortion. Result Blood flow signals were detected in 67 cases (93.06%) of 72 patients;patients showed low resistance arterial blood flow spectrum, and the resistance index (RI) values ranged from 0.36 to 0.55; the accuracy, sensitivity and specificity of CDS method were all significantly higher than urine HCG level detection (P < 0.05). Conclusion CDS is of high value in the diagnosis of incomplete medical abortion, and can provide strategies for clinical follow-up treatment.
