发散式体外冲击波不同方案治疗跖筋膜炎型跟痛症的效果对比

ObjectiveTo compare the different schemes treatment effect of radial extracorporeal shockwave treatment for plantar fasciitis type heel pain.MethodAccording to the treatment interval, 54 patients with pain syndrome were randomly divided into group 1 (interval two days), group 2 (interval four days), group 3 (interval six days). Each treatment group according to the different energy were divided into low energy group (0.08 mJ/mm2) and high energy group (0.16 mJ/mm2). Frequency at 7 Hz, 2000 pulses per time, four times in total. We used VAS and AOFAS to evaluate the effect before treatment and at 1 week, 1 month, 3 months and 6 months after treatment. Calculated the thickness of the plantar fascia under the ultrasound localization before treatment and 6 months after treatment.ResultAfter treatment, the VAS scores and AOFAS scores of the six groups were signiifcantly improved than before treatment (P<0.001). There were no signiifcant differences in the thickness of the plantar fascia before treatment and 6 months after treatment (P>0.05). After treatment there was no significant difference were observed in the comparison between the six groups (P>0.05). ConclusionRadial extracorporeal shock wave in the treatment of plantar fasciitis type with pain symptoms has good effect, but the energy and the interval number dose not have signiifcant association with the treatment effect at this text.%目的比较发散式体外冲击波不同方案治疗跖筋膜炎型跟痛症的效果。方法将54例跟痛症患者根据治疗间隔天数随机分为治疗组1（间隔2天），治疗组2（间隔4天），治疗组3（间隔6天），每个治疗组根据冲击波不同能量分为低能量组（0.08 mJ/mm2）和高能量组（0.16 mJ/mm2）。冲击波治疗频率为7 Hz，每次冲击2000次，共治疗4次。比较六组患者治疗前和治疗后1周、1个月、3个月、6个月视觉模拟评分法（VAS）评分、AOFAS踝-后足评分系统评分。分别于治疗前和治疗后6个月在超声定位下测量跖筋膜厚度。结果六组患者治疗后VAS评分、AOFAS踝-后足评分系统评分均较治疗前明显改善（P＜0.001）。六组患者治疗前跖筋膜厚度与治疗后6个月比较差异均无显著性（P＞0.05），治疗后六组间比较差异均无显著性（P＞0.05）。结论发散式体外冲击波治疗跖筋膜炎型跟痛症效果良好，但本试验显示体外冲击波能量和间隔天数与治疗效果关系不大。
