动脉自旋标记灌注成像对缺血性脑病的诊断价值

摘要

Objective To evaluate the effect and value of arterial spin labeling (ASL) perfusion imaging on ischemic encephalopathy. Method 100 patients with ischemic encephalopathy were enrolled in this study from October 2015 to August 2016 in our hospital, all patients received magnetic resonance imaging (MRI), ASL, dynamic susceptibility contrast (DSC), diffusion-weighted imaging (DWI) examination, the area of ischemic penumbra and positive detection rate were compared among different examinations, bilateral cerebral blood flow were recorded in patients with different genders under ASL perfusion imaging. Result The positive rate of ASL, DWI and DSC were higher than that of MRI (P<0.05), and the positive rate of ASL was higher than that of DSC (P<0.05). There were no significant difference in the positive rate of ASL and DWI (P>0.05). Ischemic penumbra area of DSC was larger than that of ASL (P<0.05). ASL perfusion imaging showed that there was no significant difference in cerebral blood flow among male and female patients (P > 0.05). Conclusion ASL in the diagnosis of ischemic encephavlopathy has a high value, can dynamically observe the changes in the disease, worthy of clinical promotion.%目的 评价动脉自旋标记(arterial spin labeling,ASL)灌注成像对缺血性脑病的诊断效果及价值.方法 选择本院2015年10月至2016年8月收治的100例缺血性脑病患者为研究对象,均接受常规磁共振成像(magnetic resonance imaging,MRI)、ASL、动态磁敏感对比(dynamic susceptibility contrast,DSC)、弥散加权成像(diffusion-weighted imaging,DWI)检查,比较不同检查方法的缺血半暗带面积和阳性病灶检出率,记录ASL灌注成像下不同性别患者的双侧脑血流量.结果 ASL、DWI、DSC阳性病灶检出率均高于MRI(P<0.05),ASL阳性病灶检出率显著高于DSC(P<0.05),但ASL阳性病灶检出率与DWI相比无显著差异(P>0.05).ASL缺血半暗带面积大于DSC(P<0.05);ASL灌注成像显示,男性与女性患者的患侧脑血流量无显著差异(P>0.05).结论 ASL对缺血性脑病具有较高的诊断价值,可动态观察病情变化,值得临床推广.
